目的 探討西帕依固齦液聯(lián)合氟康唑治療老年義齒性口炎的療效.方法 選取2013年1月—2013年10月中國人民解放軍第451醫(yī)院收治的義齒性口炎患者42例,隨機(jī)分為對(duì)照組和治療組,每組21例.對(duì)照組口服氟康唑片,1片/次,1次/d,首次劑量加倍.治療組在對(duì)照組基礎(chǔ)上給予西帕依固齦液含漱,5 mL/次,含漱時(shí)間3 min,3次/d.兩組均連續(xù)治療6個(gè)月.比較兩組的臨床療效,同時(shí)觀察兩組定植菌的菌群變化及口炎復(fù)發(fā)率.結(jié)果 治療后,對(duì)照組和治療組的顯效率分別為85.71%、95.24%,總有效率分別為95.24%、100.00%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05).治療15 d、治療后、復(fù)診時(shí)兩組白色念珠菌、鏈球菌、乳桿菌的菌落總數(shù)均較治療前顯著降低,同組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組白色念珠菌、乳桿菌的菌落總數(shù)少于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05).對(duì)照組和治療組復(fù)發(fā)率分別為42.86%、23.81%,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05).結(jié)論 西帕依固齦液聯(lián)合氟康唑治療老年義齒性口炎具有較好的臨床療效,可降低白色念珠菌和乳桿菌的感染,降低復(fù)發(fā)率,值得臨床推廣應(yīng)用.

Objective To investigate the clinical effect of Xipayi Mouth Rinse combined with fluconazole in treatment of denture stomatitis. Methods The patients with elderly denture stomatitis (42 cases) of the 451^th Hospital of PLAfrom January 2013 to October 2013 were randomly divided into control and treatment groups, and each group had 21 cases. The patients in the control group were po administered with Fluconazole Tablets, one tablet/time, once daily. And the dosage of first time was double. The patients in the treatment group were given Xipayi Mouth Rinse, 5 mL/time for 3 min, three times daily. The patients in two groups were treated for 6 months. After the treatment, the efficacy was evaluated, while the changes of engraftment bacteria flora and recurrence rate of stomatitis in two groups were observed. Results The significant efficiencies in the control and treatment groups were 85.71% and 95.24%, and the efficacies in two groups were 95.24% and 100.00%, respectively, with significant difference between the two groups (P < 0.05). After treatment for 15 d, after treatment, and return visit, total numbers of colonies of Candida albicans, Streptococcus, and Lactobacillus were significantly reduced before treatment, and the difference was statistically significant in the same group (P < 0.05). And numbers of colonies of C. albicans and Lactobacillus in treatment group were less than those in the control group, with significant difference between two groups (P < 0.05). The recurrence rate in control and treatment groups were 42.86% and 23.81%, respectively, with significant difference between two groups (P < 0.05). Conclusion Xipayi Mouth Rinse combined with fluconazole has the good clinical effect in treatment of elderly denture stomatitis, and can reduce the infection of C. albicans and Lactobacillus, while can reduce the recurrence rate, which can be used widely in clinic.
