目的 觀察穩(wěn)心顆粒聯(lián)合胺碘酮和貝那普利治療老年陣發(fā)性心房顫動(dòng)的臨床療效。方法 選取2013年6月-2014年6月淄博化建醫(yī)院收治的老年陣發(fā)性心房顫動(dòng)患者126例,隨機(jī)分為對(duì)照組和治療組,每組各63例。對(duì)照組患者口服鹽酸胺碘酮片,1片/次,3次/d,2周后改為2次/d;同時(shí)口服鹽酸貝那普利片,1片/次,1次/d。治療組在對(duì)照組治療基礎(chǔ)上口服穩(wěn)心顆粒,1袋/次,3次/d。兩組均連續(xù)治療12個(gè)月。觀察兩組的臨床療效,同時(shí)比較兩組患者治療前后左心房內(nèi)徑、左室收縮末期內(nèi)徑(LVSD)、左室內(nèi)徑短軸縮短率(FS)、左室射血分?jǐn)?shù)(EF)、B型納尿肽(BNP)的變化情況。結(jié)果 治療后,對(duì)照組和治療組的總有效率分別為84.13%、95.24%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療3、6、12個(gè)月時(shí),對(duì)照組左心房內(nèi)徑均較治療前顯著升高,治療12個(gè)月時(shí)對(duì)照組LVSD、BNP顯著降低,治療6、12個(gè)月時(shí)對(duì)照組FS、EF顯著升高,同治療前比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05);治療3、6、12個(gè)月時(shí),治療組左心房內(nèi)徑、FS、EF均顯著升高,LVSD、BNP顯著降低,同治療前比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05);治療組這些觀察指標(biāo)的改善程度優(yōu)于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 穩(wěn)心顆粒聯(lián)合胺碘酮和貝那普利治療老年陣發(fā)性心房顫動(dòng)具有較好的臨床療效,可顯著抑制左心房擴(kuò)大,改善患者心功能,具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To observe the clinical effect of Wenxin Granles combined with amiodarone and benazepril in treatment of elderly paroxysmal atrial fibrillation. Methods Patients (126 cases) with paroxysmal atrial fibrillation in Zibo Sinopec Construction Hospital from June 2013 to June 2014 were randomly divided into control (n=63) and treatment (n=63) groups. The patients in the control group were po administered with Amiodarone Hydrochloride Tablets, 1 tablet/time, three times daily, two weeks later twice daily instead. And they were po administered with Benazepril Hydrochloride Tablets, 1 tablet/time, once daily. The patients in the treatment group were po administered with Wenxin Granles on the basis of the control group, 1 bag/time, three times daily. The patients in two groups were treated for 12 months. After treatment, the efficacy was evaluated, and the changes of left atrium diameter, LVSD, FS, EF, and BNP in two groups before and after treatment were compared. Results After treatment, the efficacies in the control and treatment groups were 84.13% and 95.24%, respectively, and there were differences between two groups (P < 0.05). At treatment of 3, 6, and 12 months, left atrium diameter in control group were significantly increased, while LVSD and BNP in the control group were significantly decreased at treatment of 12 months, FS and EF in the control group were significantly increased at treatment for 6, 12 months, and the difference was statistically significant in the same group (P < 0.05). Left atrium diameter, FS, and EF in the treatment group were significantly increased at treatment for 3, 6, 12 months, LVSD and BNP were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). After treatment, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Wenxin Granles combined with amiodarone and benazepril has clinical curative effect in treatment of elderly paroxysmal atrial fibrillation, and can significantly inhibit the left atrial enlargement, and can improve cardiac function, which has a certain clinical application value.
