目的 探討化痰平喘片聯(lián)合氨茶堿緩釋片治療慢性阻塞性肺氣腫的臨床療效。方法 選取2014年5月-2016年1月人民解放軍海軍機關門診部呼吸內科收治的慢性阻塞性肺氣腫患者86例,根據(jù)治療方案的差別將所有患者分為對照組和治療組,每組各43例。對照組口服氨茶堿緩釋片,0.2g/次,3次/d。治療組在對照組治療基礎上口服化痰平喘片,6片/次,2次/d。兩組患者均連續(xù)治療2周。觀察兩組的臨床療效,同時比較治療前后氧分壓(pO₂)、血氧飽和度(SpO₂)、二氧化碳分壓(pCO₂)、一秒用力呼氣容積(FEV₁)、用力肺活量(FVC)、FEV₁/FVC、白細胞介素4(IL-4)、IL-8、基質金屬蛋白酶9(MMP-9)和腫瘤壞死因子α(TNF-α)的變化情況。結果 治療后,對照組和治療組的總有效率分別為83.72%、93.02%,兩組比較差異有統(tǒng)計學意義(P<0.05)。治療后,兩組患者pO₂、SpO₂、FEV₁、FVC、FEV₁/FVC均較治療前顯著升高,pCO₂、IL-4、IL-8、MMP-9、TNF-α較治療前顯著降低,同組治療前后差異有統(tǒng)計學意義(P<0.05);且治療組這些觀察指標的改善程度優(yōu)于對照組,兩組比較差異有統(tǒng)計學意義(P<0.05)。結論 化痰平喘片聯(lián)合氨茶堿緩釋片治療慢性阻塞性肺氣腫具有較好的臨床療效,可明顯改善患者的肺功能,同時還可降低血清炎性因子水平,具有一定的臨床推廣應用價值。

Objective To investigate the clinical effect of Huatan Pingchuan Tablets combined with Aminophylline Sustained-release Tablets in treatment of chronic obstructive pulmonary emphysema. Methods Patients (86 cases) with chronic obstructive pulmonary emphysema in Naval Authority Clinics of the Chinese People's Liberation Army from May 2014 to January 2016 were enrolled in this study. According to the different treatment plans, patients were divided into control group (43 cases) and treatment group (43 cases). The patients in the control group were po administered with Aminophylline Sustained-release Tablets, 0.2 g/time, three times daily. The patients in the treatment group were po administered with Huatan Pingchuan Tablets on the basis of control group, 6 tablets/time, twice daily. The patients in two groups were treated for 2 weeks. After treatment, the clinical efficacies were evaluated, and the changes of pO₂, SpO₂, pCO₂, FEV₁, FVC, FEV₁/FVC, IL-4, IL-8, MMP-9, and TNF-α in two groups before and after treatment were compared. Results After treatment, the efficacies in the control and treatment groups were 83.72% and 93.02%, respectively, and there were differences between two groups (P < 0.05). After treatment, pO₂, SpO₂, FEV₁, FVC, and FEV₁/FVC in two groups were significantly increased, and pCO₂, IL-4, IL-8, MMP-9, and TNF-α in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). After treatment, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Huatan Pingchuan Tablets combined with Aminophylline Sustained-release Tablets has clinical curative effect in treatment of chronic obstructive pulmonary emphysema, and can obviously improve the lung function, and can also reduce the levels of inflammatory cytokines, which has a certain clinical application value.
