目的 探討玉屏風(fēng)顆粒聯(lián)合鹽酸氮卓斯汀噴鼻劑治療過(guò)敏性鼻炎的臨床療效。方法 選取北京市順義區(qū)醫(yī)院2013年6月-2015年5月收治的過(guò)敏性鼻炎患者160例,隨機(jī)分成對(duì)照組和治療組,每組各80例。對(duì)照組患者給予鹽酸氮卓斯汀鼻噴霧劑治療,每側(cè)鼻孔2噴/次,每日早晚各1次。治療組在對(duì)照組基礎(chǔ)上口服玉屏風(fēng)顆粒,5g/次,3次/d。兩組患者均連續(xù)治療4周。觀察兩組患者臨床療效,比較治療前和治療2、4周后兩組的噴嚏、流涕、鼻癢、鼻堵評(píng)分、血清IgE、Th₁/Th₂水平和炎性因子白介素-4(IL-4)、IL-6、IL-10水平的變化。結(jié)果 治療后,對(duì)照組和治療組總有效率分別為78.75%、98.75%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.01)。治療2、4周后,兩組患者噴嚏、流涕、鼻癢、鼻堵評(píng)分均較治療前顯著降低,同組治療前后差異有統(tǒng)計(jì)學(xué)意義(P<0.01);且治療組的改善程度優(yōu)于同期對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.01)。兩組患者血清IgE以及IL-4、IL-6、IL-10水平均較治療前顯著降低,Th₁/Th₂水平較治療前顯著升高,同組治療前后差異有統(tǒng)計(jì)學(xué)意義(P<0.01);且治療組這些觀察指標(biāo)的改善程度優(yōu)于同期對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.01)。治療組隨訪3、6個(gè)月的復(fù)發(fā)率分別為8.75%、16.25%,顯著低于同期對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.01)。結(jié)論 玉屏風(fēng)顆粒聯(lián)合鹽酸氮卓斯汀噴鼻劑治療過(guò)敏性鼻炎的臨床療效較好,可有效降低癥狀評(píng)分,改善相關(guān)因子水平,值得臨床推廣應(yīng)用。

Objective To investigate the clinical effect of Yupingfeng Granules combined with Azelastine Hydrochloride Nasal Spray in treatment of allergic rhinitis. Methods Patients (160 cases) with allergic rhinitis in Beijing Shunyi District Hospital from June 2013 to May 2015 were randomly divided into control and treatment groups, and each group had 80 cases. The patients in the control group were given Azelastine Hydrochloride Nasal Spray both nostril 2 spouts/time, morning and evening for 1 d. The patients in the treatment group were po administered with 5 g/time, three times daily. Both groups were treated for 4 weeks. After treatment, the efficacy was evaluated, changes of sneezing, runny nose, nasal itching, and nasal congestion scores, IgE, Th₁/Th₂, inflammatory factor interleukin-4 (IL-4), IL-6, and IL-10 in two groups were compared. Results After treatment, the total effective rate of control and treatment groups were 78.75% and 98.75%, respectively, and there were differences between two groups (P < 0.01). After treatment for 2 and 4weeks, sneezing, runny nose, nasal itching, and nasal congestion scores in two groups were significantly reduced, and the difference was statistically significant in the same group (P < 0.01). And these indicators in treatment group improved better than those in the control group, with significant difference between two groups (P < 0.01). IgE, IL-4, IL-6 and IL-10 levels in two groups were significantly reduced, while Th₁/Th₂ was significantly increased, and the difference was statistically significant in the same group (P < 0.01). The improvement of detection factor in the treatment group was better than those in the control group, and there were differences between two groups (P < 0.01). The recurrence rate in treatment group after followed up for 3 and 6 months were 8.75% and 16.25%, respectively, and they were lower than those in control group at the same treatment time with significant difference (P < 0.01). Conclusion Yupingfeng Granules combined with Azelastine Hydrochloride Nasal Spray has the good clinical effect in treatment of allergic rhinitis, and can effectively reduce the symptom scores, while can adjust the rhinitis correlation factors, which is worth clinical promotion.
