目的 評(píng)估鮭魚降鈣素鼻噴劑聯(lián)合碳酸鈣D3片治療絕經(jīng)婦女橈骨遠(yuǎn)端骨折的臨床療效。方法 選取2012年12月-2014年12月在陜西省核工業(yè)二一五醫(yī)院骨科接受掌側(cè)鋼板內(nèi)固定的橈骨遠(yuǎn)端骨折絕經(jīng)婦女59例,所有患者隨機(jī)分為對(duì)照組(26例)和治療組(33例)。對(duì)照組口服碳酸鈣D3片,1片/次,1次/d。治療組在對(duì)照組治療基礎(chǔ)上給予鮭魚降鈣素鼻噴劑,200IU/d。兩組均連續(xù)治療3個(gè)月,并隨訪6個(gè)月。比較視覺模擬評(píng)分(VAS)、骨密度值、腕關(guān)節(jié)功能。結(jié)果 治療3d、1個(gè)月、3個(gè)月、隨訪6個(gè)月,兩組患者的VAS評(píng)分均較治療前顯著降低,同組治療前后比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療1、3個(gè)月后,治療組患者的VAS評(píng)分改善程度明顯優(yōu)于同期對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療3個(gè)月、隨訪6個(gè)月,治療組的骨密度值均較治療前顯著增高,同組治療前后比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組患者的骨密度值改善程度明顯優(yōu)于同期對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。而對(duì)照組的幾乎沒有變化。治療后,對(duì)照組和治療組患者腕關(guān)節(jié)功能評(píng)分優(yōu)良率分別為61.5%、87.9%,兩組優(yōu)良率比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.01)。結(jié)論 鮭魚降鈣素聯(lián)合碳酸鈣D3片治療絕經(jīng)婦女橈骨遠(yuǎn)端骨折具有較好的療效,能夠增加骨密度,促進(jìn)腕關(guān)節(jié)功能恢復(fù),具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To evaluate the clinical efficacy of Salmon Calcitonin Nasal Spray combined with Calcium Carbonate and Vitamin D3 Tablets in treatment of distal radius fracture in postmenopausal women. Methods Postmenopausal women (59 cases) with distal radius fracture in Department of Bone of No. 215 Hospital of Shaanxi Nuclear Industry from December 2012 to December 2014 were randomly divided into control (26 cases) and treatment (33 cases) groups. The patients in the control group were po administered with Calcium Carbonate and Vitamin D3 Tablets, 1 tablet/time, once daily. The patients in the treatment group were given Salmon Calcitonin Nasal Spray on the basis of the control group, 200 IU/d. The patients in two groups were treated for 3 weeks, and followed up for 6 months. VAS score, bone mineral density, and wrist joints function were compared between two groups. Results After treatment for 3 d, 1 moth, 3 months, and follow-up for 6 months, VAS score in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). After treatment for 1 moth and 3 months, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). After treatment for 3 months and follow-up for 6 months, bone mineral densities in the treatment groups were significantly increased, and the difference was statistically significant in the same group (P < 0.05). After treatment, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). But there was no change in the control group. After treatment, the excellent rates of wrist joints function in the control and treatment groups were 61.5% and 87.9%, respectively, and there were differences between two groups (P < 0.01). Conclusion Salmon Calcitonin Nasal Spray combined with Calcium Carbonate and Vitamin D3 Tablets has clinical curative effect in treatment of distal radius fracture in postmenopausal women, can relieve pain, increase bone density, and promote the wrist joint function recovery, which has a certain clinical application value.
