[摘要]
目的 研究小牛血清去蛋白聯(lián)合替羅非班治療進(jìn)展性腦梗死的臨床療效。方法 選取2014年11月—2015年11月自貢市第一人民醫(yī)院神經(jīng)內(nèi)科收治的進(jìn)展性腦梗死患者110例,隨機(jī)分為對(duì)照組和治療組,每組各55例。對(duì)照組靜脈泵入鹽酸替羅非班氯化鈉注射液,前30 min速率設(shè)定為0.4 μg/(kg·min),此后0.1μg/(kg·min)持續(xù)泵入。治療組在對(duì)照組治療基礎(chǔ)上靜脈滴注小牛血清去蛋白注射液,30 mL加入250 mL生理鹽水中,1次/d。兩組均連續(xù)治療7 d。觀察兩組的臨床療效,同時(shí)比較兩組治療前后神經(jīng)功能缺損(NIHSS)評(píng)分、Barthel指數(shù)的變化情況。結(jié)果 治療后,對(duì)照組和治療組的總有效率分別為81.82%、96.36%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組患者NIHSS評(píng)分顯著降低,Barthel指數(shù)明顯升高,同組治療前后差異均有統(tǒng)計(jì)學(xué)意義(P<0.05),且治療組這些觀察指標(biāo)的改善程度優(yōu)于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 小牛血清去蛋白聯(lián)合替羅非班治療進(jìn)展性腦梗死具有較好的臨床療效,可改善患者的神經(jīng)功能損傷程度,提高患者的生活質(zhì)量,具有一定的臨床推廣應(yīng)用價(jià)值。

[Abstract]
Objective To investigate the effect of deproteinised calf combined with tirofiban in treatment of progressive cerebral infarction. Methods Patients (110 cases) with progressive cerebral infarction in Department of Neurology of Zigong First People's Hospital from November 2014 to November 2015 were randomly divided into control and treatment groups. Each group had 55 cases. The patients in the control group were iv pumping with Tirofiban Hydrochloride Sodium Chloride Injection, and the rate was set for 0.4 μg/(kg·min) before 30 min, than 0.1 μg/(kg·min) continuous pumping. The patients in the treatment group were iv administered with Deproteinised Calf Blood Serum Injection on the basis of the control group, 30 mL added into normal saline 250 mL, once daily. The patients in two groups were treated for 7 d. After treatment, the clinical efficacies were evaluated, and the changes of NIHSS scores and Barthel index in two groups before and after treatment were compared. Results After treatment, the total efficacies in the control and treatment groups were 81.82% and 96.36%, respectively, and there were differences between two groups (P<0.05). After treatment, NIHSS scores in two groups were significantly decreased, and Barthel index was significantly increased, the difference was statistically significant in the same group (P<0.05). After treatment, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). Results Deproteinised calf combined with tirofiban has clinical curative effect in treatment of progressive cerebral infarction, and can improve the nerve function damage degree, also can improve the life quality, which has a certain clinical application value.
