目的 探討連花清瘟膠囊聯(lián)合特布他林和布地奈德治療慢性阻塞性肺疾病急性加重期的臨床療效。方法 選取2015年5月—2016年1月在仙桃市中醫(yī)醫(yī)院呼吸內(nèi)科進(jìn)行治療的慢性阻塞性肺疾病急性加重期患者80例，按照治療方法的不同分為對照組和治療組，每組各40例。對照組給予硫酸特布他林霧化液5 mg聯(lián)合吸入用布地奈德混懸液1 mg與生理鹽水10mL混合后進(jìn)行霧化吸入，20 min/次，2次/d。治療組在對照組治療基礎(chǔ)上口服連花清瘟膠囊，4粒/次，3次/d。兩組患者均連續(xù)治療14 d。觀察兩組的臨床療效，同時(shí)比較兩組喘息、咳嗽、哮鳴音等臨床癥狀改善時(shí)間；比較兩組患者治療前后病情積分、一秒用力呼氣容積（FEV₁）、用力肺活量（FVC）、FEV₁/FVC、腫瘤壞死因子α（TNF-α）、白細(xì)胞介素6（IL-6）、IL-8、pH值、血氧分壓（pO₂）以及動脈血二氧化碳分壓（pCO₂）的變化情況。結(jié)果 治療后，對照組和治療組的總有效率分別為80.0%、95.0%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，治療組患者喘息緩解時(shí)間、咳嗽消失時(shí)間、哮鳴音消失時(shí)間均顯著短于對照組，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者病情積分、pCO₂、IL-6、IL-8、TNF-α均顯著降低，pH值、pO₂、FEV₁、FVC、FEV₁/FVC均顯著升高，同組治療前后差異有統(tǒng)計(jì)學(xué)意義（P<0.05），且治療組這些觀察指標(biāo)的改善程度優(yōu)于對照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 連花清瘟膠囊聯(lián)合特布他林和布地奈德治療慢性阻塞性肺疾病急性加重期具有較好的臨床療效，可明顯改善患者的臨床癥狀和肺功能，并可降低機(jī)體炎性反應(yīng)，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical effect of Lianhua Qingwen Capsules combined with terbutaline and budesonide in treatment of acute exacerbation of chronic obstructive pulmonary disease. Methods Patients (80 cases) with acute exacerbation of chronic obstructive pulmonary disease in Traditional Chinese Medicine Hospital of Xiantao from May 2015 to January 2016 were enrolled in this study. According to the different treatment plans, patients were divided into control group (40 cases) and treatment group (40 cases). The patients in the control group were inhal administered with Terbutaline Sulphate Solution for nebulization 5 mg and Budesonide Suspension for inhalation 1 mg added into normal saline 10 mL, 20 mL/time, twice daily. The patients in the treatment group were po administered with Lianhua Qingwen Capsules on the basis of the control group, 4 grains/time, three times daily. The patients in two groups were treated for 14 d. After treatment, the clinical efficacies were evaluated, improved time of wheezing, coughing, wheezing sound in two groups were compared. The changes of illness integration, FEV₁, FVC, FEV₁/FVC, TNF-α, IL-6, IL-8, pH value, pO₂, and pCO₂ in two groups before and after treatment were compared. Results After treatment, the total efficacies in the control and treatment groups were 80.0% and 95.0%, respectively, and there were differences between two groups (P<0.05).After treatment, breathing ease time, coughing disappeared time, and wheezing sound disappeared time in the treatment group were shorter than those in the control group. After treatment, illness integration, pCO₂, IL-6, IL-8, and TNF-α in two groups were significantly decreased, and pH value, pO₂, FEV₁, FVC, and FEV₁/FVC were significantly increased, the difference was statistically significant in the same group (P<0.05). After treatment, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). Conclusion Lianhua Qingwen Capsules combined with terbutaline and budesonide has clinical curative effect in treatment of acute exacerbation of chronic obstructive pulmonary disease, and can obviously improve the clinical symptoms and lung function and reduce the inflammatory response, which has a certain clinical application value.
