目的 探討欖香烯注射液聯(lián)合培美曲塞和奈達(dá)鉑治療晚期肺癌的臨床療效。方法 選取2014年2月—2016年2月在通山縣人民醫(yī)院接受治療的晚期肺癌患者86例，按照治療方法的不同分成對照組（43例）和治療組（43例）。兩組患者均給予必要的基礎(chǔ)治療。對照組靜脈滴注注射用培美曲塞二鈉，0.5 mg/m2；同時靜脈滴注注射用奈達(dá)鉑，75 mg/m2，每21 d給藥1次，21 d為一個療程。治療組在對照組基礎(chǔ)靜脈滴注欖香烯注射液，0.6 g加入5%葡萄糖溶液500 mL，1次/d，21 d為一個療程，休息1周后進(jìn)行下一療程。兩組患者均連續(xù)治療2個療程。觀察兩組的臨床療效，比較兩組治療前后生存質(zhì)量、免疫功能和血清基質(zhì)金屬蛋白酶（MMP）水平的改善情況。結(jié)果 治療后，對照組和治療組客觀緩解率（ORR）分別為55.81%和76.74%，兩組臨床獲益率（CBR）分別為65.12%和83.72%，兩組近期臨床療效比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，對照組和治療組生存質(zhì)量改善率分別為81.40%和95.35%，兩組患者改善率比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組患者CD³⁺、CD⁴⁺、CD⁴⁺/CD⁸⁺均明顯升高，同組比較差異具有統(tǒng)計學(xué)意義（P<0.05），且治療組升高的更明顯，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組患者血清MMP-2和MMP-9水平均較同組治療前明顯降低（P<0.05），且治療組降低的更顯著（P<0.05）。結(jié)論欖香烯注射液聯(lián)合培美曲塞和奈達(dá)鉑治療晚期肺癌具有很好的臨床療效，可以提高患者免疫能力及改善生活質(zhì)量，具有一定的臨床應(yīng)用推廣價值。

Objective To explore the clinical effect of Elemene Injection combined with pemetrexed and nedaplatin in treatment of advanced lung cancer. Methods Patients (86 cases) with advanced lung cancer in the People's Hospital of Tongshan from February 2014 to February 2016 were divided into control group (43 cases) and treatment group (43 cases) according to the different treatments. All patients were given necessary basic treatment. The patients in the control group were iv administered with Pemetrexed Disodium for injection, 0.5 mg/m²; And they were iv administered with Nedaplatin for injection, 75 mg/m², once every 21 d, and 21 d for a course of treatment. The patients in the treatment group were iv administered with Elemene Injection on the basis of control group, 0.6 g added into 5% glucose solution 500 mL, once daily, 21 d for a course of treatment, and were repeated the next course of treatment after 1 week's rest. All patients were treated for two courses of treatment. After treatment, the clinical short-term effect was evaluated, and the improvement of survival quality, immune function, and serum MMP levels in two groups were compared. Results After treatment, ORR in the control group and treatment group were 55.81% and 76.74%, respectively, and CBR in two groups were 65.12% and 83.72%, and there were differences between two groups (P<0.05). After treatment, the improvement rates of survival quality in the control group and treatment group were 81.40% and 95.35%, respectively, and there were differences between two groups (P<0.05). After treatment, CD³⁺, CD⁴⁺, and CD⁴⁺/CD⁸⁺ in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05), and the treatment group increased more significantly, with significant difference between two groups (P<0.05). After treatment, serum MMP-9 and MMP-2 levels in two groups were significantly decreased (P<0.05), and the treatment group decreased more significantly (P<0.05). Conclusion Elemene injection combined with pemetrexed and nedaplatin has a good clinical curative effect in treatment of advanced lung cancer, can enhance the immune function and improve the survival quality, which has a certain clinical application value.
