目的 探討磷酸鋁凝膠聯(lián)合蘭索拉唑治療幽門螺桿菌（Hp）陽(yáng)性胃潰瘍的臨床療效。方法 選取2015年10月-2016年10月在鄭州市中醫(yī)院接受治療的Hp陽(yáng)性胃潰瘍患者82例，按照治療方法的差別分成對(duì)照組（41例）和治療組（41例）。對(duì)照組口服蘭索拉唑口崩片，1片/次，1次/d。治療組在對(duì)照組的基礎(chǔ)上口服磷酸鋁凝膠，1袋/次，3次/d。兩組患者均連續(xù)治療4周。觀察兩組患者臨床療效，同時(shí)比較治療前后兩組患者Hp清除率、臨床癥狀評(píng)分、超敏C反應(yīng)蛋白（hs-CRP）、基質(zhì)金屬蛋白酶-9（MMP-9）及胃泌素水平變化。結(jié)果 治療后，對(duì)照組和治療組的總有效率分別為80.49%和97.56%，兩組總有效率比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者腹脹、腹痛及反酸噯氣臨床癥狀評(píng)分均較同組治療前降低，同組比較具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組患者臨床癥狀評(píng)分比對(duì)照組降低的更明顯，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者血清hs-CRP、MMP-9、胃泌素水平明顯低于同組治療前，同組比較具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組上述指標(biāo)水平顯著優(yōu)于對(duì)照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，治療組患者Hp清除率為90.24%，顯著高于對(duì)照組的73.17%，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 磷酸鋁凝膠聯(lián)合蘭索拉唑治療Hp陽(yáng)性胃潰瘍臨床效果顯著，可明顯降低機(jī)體hs-CRP、MMP-9及胃泌素水平，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical efficacy of Aluminium Phosphate Gel combined with lansoprazole in treatment of helicobacter pylori (Hp) positive gastric ulcer. Methods Patients (82 cases) with Hp positive gastric ulcer in Zhengzhou Hospital of TCM from October 2015 to October 2016 were divided into control (41 cases) and treatment (41 cases) groups based on different treatments. Patients in control group were po administered with Lansoprazole Orally Disintegrating Tablets, 1 tablet/time, once daily. Patients in treatment group were po administered with Aluminium Phosphate Gel on the basis of the control group, 1 bag/time, three times daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacies were evaluated, and the Hp clearance rate, clinical symptom score, hs-CRP, MMP-9, and gastrin levels in two groups before and after treatment were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 80.49% and 97.56%, respectively, and there were differences between two groups (P<0.05). After treatment, clinical symptom score of abdominal distension, abdominal pain, acid regurgitation and belching in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the clinical symptom score in the treatment group decreased more significantly than that in the control group, with significant difference between two groups (P<0.05). After treatment, hs-CRP, MMP-9, and gastrin level in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And these indicators in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the Hp clearance rate in treatment group was 90.24%, which was significantly higher than that (73.17%) in the control group, and the difference was statistically significant between two groups (P<0.05). Conclusion Aluminium Phosphate Gel combined with lansoprazole has a significant clinical effect in treatment of helicobacter pylori positive gastric ulcer, can significantly decreased the hs-CRP, MMP-9 and gastrin level, which has a certain clinical application value.
