[摘要]
目的 探討小牛血清去蛋白注射液聯(lián)合奧扎格雷鈉氯化鈉注射液治療老年急性腦梗死的臨床療效。方法 選取2014年10月-2016年1月昆山市第一人民醫(yī)院收治的老年急性腦梗死患者81例,隨機分為對照組(40例)和研究組(41例)。對照組靜脈滴注奧扎格雷鈉氯化鈉注射液,80 mg加入到250 mL生理鹽水中,1次/d。研究組在對照組基礎上靜脈滴注小牛血清去蛋白注射液,30 mL加入到250 mL生理鹽水中,1次/d。兩組患者均治療15 d。觀察兩組的臨床療效,比較兩組的美國國立衛(wèi)生研究院卒中量表(NIHSS)評分、同型半胱氨酸(Hcy)、S100B蛋白(S100B)和基質金屬蛋白酶-8(MMP-8)水平以及改良Rankin量表(mRS)評分。結果 治療后,對照組和治療組的總有效率分別為77.50%、92.68%,兩組總有效率比較差異有統(tǒng)計學意義(P<0.05)。治療后,兩組NIHSS評分均明顯下降,同組治療前后比較差異有統(tǒng)計學意義(P<0.05);且治療組NIHSS評分的下降程度明顯優(yōu)于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。治療后,兩組Hcy、S100B、MMP-8水平均顯著降低,同組治療前后比較差異具有統(tǒng)計學意義(P<0.05);且治療組這些觀察指標的下降程度明顯優(yōu)于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。治療后90 d,治療組的mRS評分明顯小于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。結論 小牛血清去蛋白注射液聯(lián)合奧扎格雷鈉氯化鈉注射液治療老年腦梗死具有較好的臨床療效,能改善腦神經(jīng)功能及預后,調節(jié)Hcy、S100B和MMP-8的水平,具有一定的臨床推廣應用價值。

Objective To investigate the clinical effect of Deproteinised Calf Blood Serum Injection combined with Sodium Ozagrel and Sodium Chloride Injection in treatment of senile acute cerebral infarction. Methods Patients (81 cases) with senile acute cerebral infarction in the First People's Hospital of Kunshan from October 2014 to January 2016 were randomly divided into control group (40 cases) and treatment group (41 cases). Patients in the control group were iv administered with Sodium Ozagrel and Sodium Chloride Injection, 80 mg added into normal saline 250 mL, once daily. Patients in the treatment group were iv administered with Deproteinised Calf Blood Serum Injection on the basis of the control group, 30 mL added into normal saline 250 mL, once daily. Patients in two groups were treated for 15 d. After treatment, the clinical efficacies were evaluated, and levels of Hcy, S100B, and MMP-8, NIHSS scores, and mRS scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 77.50% and 92.68%, respectively, and there was difference between two groups (P<0.05). After treatment, the NIHSS scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the NIHSS score in the treatment group was significantly lower than that in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of Hcy, S100B, and MMP-8 in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment for 90 d, the mRS scores in the treatment group were lower than those in the control group, and there was difference between two groups (P<0.05). Conclusion Deproteinised Calf Blood Serum Injection combined with Sodium Ozagrel and Sodium Chloride Injection has clinical curative effect in treatment of senile acute cerebral infarction, can improve brain function and prognosis, and regulate the levels of Hcy, S100B, and MMP-8, which has a certain clinical application value.
