目的 探討唑來膦酸注射液聯(lián)合碳酸鈣D₃片治療老年女性骨質疏松的臨床療效。方法 選取2014年6月-2015年8月在保定市第一中心醫(yī)院治療的老年女性骨質疏松患者110例，所有患者隨機分為對照組和治療組，每組各55例。對照組口服碳酸鈣D₃片，2片/次，1次/d。治療組在對照組基礎上靜脈滴注唑來膦酸注射液，4 mg加入到0.9%氯化鈉溶液100mL中，1次/年。兩組患者均治療1年。觀察兩組的臨床療效，比較兩組的骨密度、骨代謝指標和VAS評分情況。結果 治療后，對照組和治療組的總有效率分別為74.5%、90.9%，兩組比較差異有統(tǒng)計學意義（P<0.05）。治療后，兩組患者腰椎骨、股骨頸和Ward's三角區(qū)骨密度均明顯升高，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療組這些觀察指標的升高程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組堿性磷酸酶（ALP）的水平明顯下降，治療組鈣（Ca）、磷（P）和降鈣素（HCT）水平明顯升高，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療組這些觀察指標的改善程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組VAS評分均明顯下降，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療組VAS評分低于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。結論 唑來膦酸注射液聯(lián)合碳酸鈣D₃片治療老年女性骨質疏松具有較好的臨床療效，能增加患者骨密度，調(diào)節(jié)骨代謝指標，減少疼痛程度，具有一定的臨床推廣應用價值。

Objective To investigate the clinical effect of Zoledronic Acid Injection combined with Calcium Carbonate and Vitamin D₃ Tablets in treatment of osteoporosis in old women. Methods Elderly female patients (110 cases) with osteoporosis in the First Central Hospital of Baoding from June 2014 to August 2015 were randomly divided into control and treatment groups, and each group had 55 cases. Patients in the control group were po administered with Calcium Carbonate and Vitamin D₃ Tablets, 2 tablets/time, once daily. Patients in the treatment group were iv administered with Zoledronic Acid Injection on the basis of the control group, 4 mg added into normal saline 100 mL, once yearly. Patients in two groups were treated for one year. After treatment, the clinical efficacies were evaluated, and bone density, bone metabolism indexes, and VAS scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 74.5% and 90.9%, respectively, and there was difference between two groups (P<0.05). After treatment, the bone density of lumbar spine, femoral neck, and Ward's Triangle in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly higher than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of ALP in two groups were significantly decreased, but the levels of Ca, P, and HCT in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the VAS scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the VAS score in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Zoledronic Acid Injection combined with Calcium Carbonate and Vitamin D₃ Tablets has clinical curative effect in treatment of osteoporosis in old women, can increase bone density, regulate bone metabolism index, and reduce the pains, which has a certain clinical application value.
