目的 探討腎衰寧顆粒聯(lián)合沙格列汀片治療糖尿病腎病的的臨床療效。方法 選取2015年9月-2016年8月云南省第二人民醫(yī)院收治的糖尿病腎病患者70例，按照隨機(jī)數(shù)字表法分為對(duì)照組和治療組，每組各35例。對(duì)照組口服沙格列汀片，5 mg/次，1次/d。治療組在對(duì)照組基礎(chǔ)上口服腎衰寧顆粒，5 g/次，3次/d。兩組均連續(xù)治療8周。觀察兩組的臨床療效，比較兩組血糖、腎功能和炎癥因子的情況。結(jié)果 治療后，對(duì)照組和治療組的總有效率分別為65.71%、88.57%，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組空腹血糖（FBG）、糖化血紅蛋白（HbA1c）、血尿素氮（BUN）、尿白蛋白排泄率（UAER）和血清胱抑素C（Cys-C）均顯著降低，同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組BUN、UAER和Cys-C的下降程度明顯優(yōu)于對(duì)照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組腫瘤壞死因子-α（TNF-α）、白細(xì)胞介素-6（IL-6）和白細(xì)胞介素-18（IL-18）均顯著降低，同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)的下降程度明顯優(yōu)于對(duì)照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 腎衰寧顆粒聯(lián)合沙格列汀片治療糖尿病腎病具有較好的臨床療效，能顯著改善患者腎功能，降低血糖，減輕炎癥反應(yīng)，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical effect of Shenshuaining Granules combined with Saxagliptin Tablets in treatment of diabetic nephropathy. Methods Patients (70 cases) with diabetic nephropathy in Yunnan Second People's Hospital from September 2015 to August 2016 were randomly divided into control and treatment groups, and each group had 35 cases. Patients in the control group were po administered with Saxagliptin Tablets, 5 mg/time, once daily. Patients in the treatment group were po administered with Shenshuaining Granules on the basis of the control group, 5 g/time, three times daily. Patients in two groups were treated for 8 weeks. After treatment, the clinical efficacies were evaluated, and blood glucose, renal function, and inflammatory factors in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 65.71% and 88.57%, respectively, and there was difference between two groups (P < 0.05). After treatment, FBG, HbA1c, BUN, UAER, and Cys-C in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). BUN, UAER, and Cys-C in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). After treatment, TNF-α, IL-6, and IL-18 in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). The observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Shenshuaining Granules combined with Saxagliptin Tablets has clinical curative effect in treatment of diabetic nephropathy, can significantly improve renal function, decrease blood glucose, and relieve inflammatory reaction, which has a certain clinical application value.
