目的 探討胃乃安膠囊聯(lián)合奧美拉唑治療十二指腸潰瘍的臨床療效。方法 收集2016年1月—2017年1月在中國人民解放軍第一八八醫(yī)院接受治療的十二指腸潰瘍患者76例，隨機(jī)分為對(duì)照組（38例）和治療組（38例）。對(duì)照組患者口服奧美拉唑腸溶膠囊，20 mg/次，1次/d。治療組在對(duì)照組的基礎(chǔ)上口服胃乃安膠囊，4粒/次，3次/d。兩組患者均治療4周。評(píng)價(jià)治療后兩組患者臨床療效，比較治療前后兩組臨床癥狀消失時(shí)間、血清細(xì)胞性因子和血清胃腸激素水平變化。結(jié)果 治療后，對(duì)照組總有效率為81.58%，顯著低于治療組的97.37%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P< 0.05）。治療后，治療組潰瘍痛、燒心、反酸消失時(shí)間均比對(duì)照組明顯縮短，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P< 0.05）。治療后，兩組患者血清IL-8、腫瘤壞死因子-α（TNF-α）、高遷移率蛋白族1（HMGB1）水平均明顯低于同組治療前（P< 0.05）；且治療組上述血清細(xì)胞性因子水平明顯低于對(duì)照組（P< 0.05）。治療后，兩組患者血清腎上腺髓質(zhì)素（AM）、胃腸激素胃泌素（GAS）水平明顯降低，降鈣素基因相關(guān)肽（CGRP）和生長抑素（SS）水平明顯升高，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P< 0.05）；且治療組上述血清胃腸激素指標(biāo)水平明顯優(yōu)于對(duì)照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P< 0.05）。結(jié)論 胃乃安膠囊聯(lián)合奧美拉唑治療十二指腸潰瘍的臨床效果明確，可明顯降低血清炎性因子及胃腸激素水平，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical effect of Weinaian Capsuls combined with omeprazole in treatment of duodenal ulcer. Methods Patients (76 cases) with duodenal ulcer in No.188 Hospital of PLA from January 2016 to January 2017 were randomly divided into control (38 cases) and treatment (38 cases) groups. Patients in the control group were po administered with Omperazole Enteric-coated Capsules, 20 mg/time, once daily. Patients in the treatment group were po administered with Weinaian Capsuls on the basis of the control group, 4 grains/time, three times daily. Patients in two groups were treated for 4 weeks. After treatment, clinical efficacy was evaluated, and the disappearance time of clinical symptom, the changes of serum cytokine and serum gastrointestinal hormones levels in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 81.58%, which was significantly lower than 97.37% in the treatment group, and there were differences between two groups (P < 0.05). After treatment, the clinical symptom disappearance time of ulcer pain, heartburn, acid reflux in the treatment group was significantly shorter than that in the control group, with significant difference between two groups (P < 0.05). After treatment, the IL-8, TNF-α and HMGB1 levels in two groups were significantly decreased (P < 0.05). And the serum cytokine levels in the treatment group was significantly lower than those in the control group (P < 0.05). After treatment, the AM and GAS levels in two groups were significantly decreased, but CGRP and SS levels were significantly increased, and the difference was statistically significant in the same group (P < 0.05). And the gastrointestinal hormones levels in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Weinaian Capsuls combined with omeprazole has clear clinical effect in treatment of duodenal ulcer, can significantly reduce the serum inflammatory factors and gastrointestinal hormones levels, which has a certain clinical application value.
