目的 探討銀杏葉提取物注射液聯(lián)合坎地沙坦治療慢性充血性心力衰竭的臨床療效。方法 選取2014年2月-2017年2月在商洛市中心醫(yī)院就診的慢性充血性心力衰竭患者98例，隨機(jī)分為對(duì)照組和治療組，每組各49例。對(duì)照組口服坎地沙坦酯片，2片/次，1次/d。治療組在對(duì)照組的基礎(chǔ)上靜脈滴注銀杏葉提取物注射液，20 mL加入5%葡萄糖注射液100 mL，糖尿病患者使用生理鹽水100 mL，1次/d。兩組患者均治療4周。評(píng)價(jià)兩組患者臨床療效，同時(shí)比較治療前后兩組患者超聲心電圖指標(biāo)和6 min步行距離變化。結(jié)果 治療后，對(duì)照組和治療組的臨床總有效率分別為75.51%、93.88%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者心電圖指標(biāo)每搏輸出量（SV）、左室射血分?jǐn)?shù)（LVEF）、每分鐘心臟輸出量（CO）和心臟指數(shù)（CI）均顯著升高，左室舒張末內(nèi)徑（LVEDD）顯著降低，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組超聲心電圖指標(biāo)較對(duì)照組改善更明顯，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者6 min步行距離均明顯增加（P<0.05）；且治療組患者6 min步行距離明顯大于對(duì)照組（P<0.05）。結(jié)論 銀杏葉提取物注射液聯(lián)合坎地沙坦能逆轉(zhuǎn)慢性充血性心力衰竭患者心臟重塑，改善運(yùn)動(dòng)功能，安全性高，具有一定的臨床推廣應(yīng)用價(jià)值。

Objection To explorer the therapeutic effect of Extract of Ginkgo biloba Leaves Injection combined with candesartan in treatment of chronic congestive heart failure. Methods Patients (98 cases) with chronic congestive heart failure in Shangluo Central Hospital from February 2014 to February 2017 were randomly divided into control and treatment groups, and each group had 49 cases. Patients in the control group were po administered with Candesartan Cilexetil Tablets, 2 tablets/time, once daily. Patients in the treatment group were iv administered with Extract of Ginkgo biloba Leaves Injection on the basis of the control group, 20 mL added into 5% glucose injection 100 mL, and diabetics used normal saline 100 mL instead, once daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacy was evaluated, and the change of ultrasonic cardiogram indexes and 6 min walking distance in two groups before and after treatment was compared. Results After treatment, the clinical efficacy in the control and treatment groups were 75.51% and 93.88%, respectively, and there were differences between two groups (P<0.05). After treatment, the ultrasonic cardiogram indexes of SV, LVEF, CO and CI in two groups were significantly increased, but LVEDD was significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the improvement of ultrasonic cardiogram indexes in the treatment group was more obviously than that in the control group, with significant difference between two groups (P<0.05). After treatment, the 6 min walking distance in two groups was significantly increased (P<0.05). And the 6 min walking distance in the treatment group was significantly longer than that in the control group (P<0.05). Conclusion Extract of Ginkgo biloba Leaves Injection combined with candesartan can reverse cardiac remodeling in treatment of chronic congestive heart failure, and improve motor function with high safety, which has a certain clinical application value.
