目的 探討夏枯草口服液聯(lián)合復(fù)方倍他米松治療亞急性甲狀腺炎的臨床療效。方法 收集2016年1月-2017年1月在棗莊礦業(yè)集團(tuán)中心醫(yī)院進(jìn)行診治的亞急性甲狀腺炎患者86例，根據(jù)治療方法的差別分為對照組（43例）和治療組（43例）。對照組患者于甲狀腺腫大明顯處氣管旁斜放射狀注射復(fù)方倍他米松注射液，每側(cè)5 mg，第1、2周均注射2次，第3周注射1次。治療組在對照組的基礎(chǔ)上口服夏枯草口服液，10 mL/次，2次/d。兩組患者均治療3周。評價(jià)兩組患者臨床療效，同時(shí)比較治療前后兩組患者臨床癥狀改善情況、血清甲狀腺功能和血清學(xué)指標(biāo)。結(jié)果 治療后，對照組和治療組總有效率分別為81.40%、97.67%；兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，治療組退熱時(shí)間、疼痛消失時(shí)間和腫塊消失時(shí)間均明顯短于對照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組血清游離三碘狀腺素（FT₃）和游離甲狀腺素（FT₄）水平均顯著降低，促甲狀腺素（TSH）水平均顯著升高，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組血清甲狀腺功能指標(biāo)改善顯著優(yōu)于對照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者血清C反應(yīng)蛋白（CRP）、腫瘤壞死因子-α（TNF-α）和胰島素樣生長因子-1（IGF-1）水平均顯著降低（P<0.05）；且治療組血清學(xué)指標(biāo)比對照組降低的更明顯（P<0.05）。結(jié)論 夏枯草口服液聯(lián)合地塞米松治療亞急性甲狀腺炎可有效改善患者臨床癥狀和甲狀腺功能，并可降低機(jī)體炎癥反應(yīng)，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical efficacy of Xiakucao Oral Liquid combined with compound betamethasone in treatment of subacute thyroiditis. Methods Patients (86 cases) with subacute thyroiditis in Zaozhuang Mining Group Central Hospital from January 2016 to January 2017 were divided into control (43 cases) and treatment (43 cases) groups based on different treatments. Patients in the control group were administered with Compound Betamethasone Injection by radial injection at the location of oblique trachea of obvious thyroid swollen, 5 mg each side, twice a week at the first and second weeks, then once at the third week. Patients in the treatment group were po administered with Xiakucao Oral Liquid on the basis of the control group, 10 mL/time, twice daily. Patients in two groups were treated for 3 weeks. After treatment, the clinical efficacy was evaluated, and the improvement of clinical symptom, serum thyroid function, and serological markers in two groups before and after treatment was compared. Results After treatment, the clinical efficacy in the control and treatment groups were 81.40% and 97.67%, respectively, and there were differences between two groups (P<0.05). After treatment, the disappearance time of fever, pain and lump in treatment group were significantly shorter than that in the control group, with significant difference between two groups (P<0.05). After treatment, the FT3 and FT4 levels in two groups were significantly decreased, but TSH was significantly increased, and the difference was statistically significant in the same group (P<0.05). And the serum thyroid function in the treatment group was significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the CRP, TNF-α and IGF-1 levels in two groups were significantly decreased (P<0.05). And the serological markers in the treatment group were significantly lower than those in the control group (P<0.05). Conclusion Xiakucao Oral Liquid combined with compound betamethasone can effectively improve the clinical symptoms and thyroid function in treatment of subacute thyroiditis, and can reduce inflammatory response, which has a certain clinical application value.
