目的 探討天芪降糖膠囊聯(lián)合羥苯磺酸鈣治療早期糖尿病腎病的臨床療效。方法 選取2016年2月-2017年3月寶雞市人民醫(yī)院收治的144例早期糖尿病腎病患者，隨機分為對照組和治療組，每組各72例。對照組口服羥苯磺酸鈣膠囊，1粒/次，3次/d。治療組在對照組治療基礎上口服天芪降糖膠囊，5粒/次，3次/d。兩組患者均連續(xù)治療2個月。觀察兩組的臨床療效，比較兩組治療前后尿蛋白、腎功能指標、血清炎性因子的變化情況。結果 治療后，對照組和治療組的總有效率分別為77.78%、91.67%，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組24 h尿微量白蛋白（24 h UMA）、白蛋白排泄率（UAER）、血肌酐（Scr）、尿素氮（BUN）、白細胞介素-6（IL-6）、腫瘤壞死因子-α（TNF-α）、高敏C反應蛋白（hs-CRP）水平均顯著下降，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療后，治療組24 h UMA、UAER、Scr、BUN、IL-6、TNF-α、hs-CRP水平低于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。結論 天芪降糖膠囊聯(lián)合羥苯磺酸鈣治療早期糖尿病腎病，可顯著降低尿蛋白，改善腎功能指標，緩解機體炎癥反應，具有一定的臨床推廣應用價值。

Objective To observe the clinical curative effect of Tianqi Jiangtang Capsules combined with calcium dobesilate in treatment of early diabetic nephropathy. Methods Patients (144 cases) with early diabetic nephropathy in Baoji People's Hospital from February 2016 to March 2017 year were randomly divided into control and treatment groups, and each group had 72 cases. Patients in the control group were po administered with Calcium Dobesilate Capsules, 1 grain/time, three times daily. Patients in the treatment group were po administered with Tianqi Jiangtang Capsules on the basis of the control group, 5 grains/time, three times daily. Patients in two groups were treated for 2 months. After treatment, the clinical efficacy was evaluated, and the changes of urinary protein, renal function indexes and serum inflammatory factors in two groups were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 77.78% and 91.67%, respectively, and there was difference between two groups (P<0.05). After treatment, 24 h UMA, UAER, Scr, BUN, IL-6, TNF-α, and hs-CRP in two groups significantly decreased, and the difference was statistically significant in the same group (P<0.05). The levels of 24 h UMA, UAER, Scr, BUN, IL-6, TNF-α and hs-CRP in the treatment group were lower than those in the control group, and there was difference between two groups (P<0.05). Conclusion Tianqi Jiangtang Capsules combined with calcium dobesilate has a significant effect in treatment of early diabetic nephropathy, and can significantly reduce urinary protein, improve renal function index, also can relieve the inflammatory response, which has a certain clinical application value.
