[摘要]
目的 探討魚腥草注射液聯(lián)合利奈唑胺治療耐甲氧西林金黃色葡萄球菌(MRSA)感染肺炎的臨床療效。方法 選取2014年5月-2016年5月在鄭州市第六人民醫(yī)院接受治療的78例MRSA感染肺炎患者為研究對象,根據(jù)用藥方案的不同分為對照組(39例)和治療組(39例)。對照組靜脈注射利奈唑胺注射液,600 mg/次,2次/d。治療組在對照組的基礎上靜脈滴注魚腥草注射液,100 mL加入5%葡萄糖溶液500 mL,1次/d。兩組均治療2周。評價兩組患者臨床療效,同時比較治療前后兩組患者MRSA清除率、臨床癥狀緩解時間和血清學指標的變化。結(jié)果 治療后,對照組和治療組的總有效率分別為79.49%、94.87%,兩組比較差異具有統(tǒng)計學意義(P<0.05)。對照組和治療組的MRSA清除率分別為74.36%、92.31%,兩組MRSA清除率比較差異具有統(tǒng)計學意義(P<0.05)。治療后,治療組的體溫恢復正常時間、咳嗽咳痰緩解時間、胸悶、氣促緩解時間明顯短于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。兩組C反應蛋白(CRP)、腫瘤壞死因子-α(TNF-α)、降鈣素(PCT)、總氧化態(tài)(TOS)水平明顯降低,而總抗氧化態(tài)(TAS)和對氧磷酶1(PON1)水平明升高,同組治療前后差異有統(tǒng)計學意義(P<0.05);且治療組的血清學指標改善程度優(yōu)于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。結(jié)論 魚腥草注射液聯(lián)合利奈唑胺治療MRSA感染肺炎效果顯著,可有效改善患者臨床癥狀,降低機體炎癥反應,有利于改善機體氧化應激狀態(tài),具有較好的臨床應用價值。

Objective To investigate the clinical efficacy of Yuxingcao Injection combined with linezolid in treatment of pneumonia infected by methicillin resistant staphylococcus aureus (MRSA). Methods Patients (78 cases) with pneumonia infected by MRSA in Zhengzhou Sixth People's Hospital from May 2014 to May 2016 were divided into control (39 cases) and treatment (319 cases) groups based on different treatments. Patients in the control group were iv administered with Linezolid Injection, 600 mg/time, twice daily. Patients in the treatment group were iv administered with Yuxingcao Injection on the basis of the control group, 100 mgL added into 5% glucose solution, once daily. Patients in two groups were treated for 2 weeks. After treatment, clinical efficacy was evaluated, and the MRSA clearance rate, relief time of clinical symptom, and serum indexes in two groups before and after treatment were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 79.49% and 94.87%, respectively, and there was difference between two groups (P<0.05). MRSA clearance rate in the control and treatment groups were 74.36% and 92.31%, respectively, and there was difference between two groups (P<0.05). After treatment, temperature recovery time, relief time of cough, sputum, chest stufly, and polypnea in the treatment group were shorter than those in the control group, with significant difference between two groups (P<0.05). The levels of CRP, TNF-α, PCT, and TOS in two groups were significantly decreased, but the TAS and PON1 in two groups was significantly increased, and the difference was statistically significant in the same group (P<0.05). And the serum indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). Conclusion Yuxingcao Injection combined with linezolid has significant effect in treatment of pneumonia infected by MRSA, can effectively improve the clinical symptoms, reduce inflammation, improve the oxidative stress state of organism, which has good clinical application value.
