目的 探討巴曲酶注射液聯(lián)合地塞米松磷酸鈉注射液治療突發(fā)性耳聾的臨床療效。方法 選取2014年6月-2016年7月南京醫(yī)科大學第二附屬醫(yī)院收治的突發(fā)性耳聾患者60例為研究對象，根據(jù)隨機分組法將60例患者分為對照組和治療組，每組各30例。對照組采用鼓室內(nèi)注射地塞米松磷酸鈉注射液，5 mg/次，3次/周。治療組在對照組基礎上靜脈滴注巴曲酶注射液，10 BU加入到生理鹽水100 mL中，1次/2 d，自第2天復查凝血指標，若纖維蛋白原（FIB）>1.0，則用量減半。兩組患者均連續(xù)治療7 d。觀察兩組的臨床療效，比較兩組不同程度耳聾有效率、凝血功能指標和血流動力學指標。結(jié)果 治療后，對照組和治療組的總有效率分別為70.0%、83.3%，兩組比較差異有統(tǒng)計學意義（P<0.05）。治療后，治療組中度、中重度、重度耳聾患者治療有效率均顯著高于對照組，兩組比較差異有統(tǒng)計學意義（P<0.05）。治療后，兩組凝血酶時間（TT）、凝血酶原時間（PT）、活化部分凝血酶原時間（ATPP）均明顯上升，而FIB均顯著下降，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療組這些觀察指標的改善程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組血漿黏度（PV）、全血黏度高切黏度（HS）、中切黏度（MS）和低切黏度（LS）均明顯降低，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療組這些觀察指標明顯低于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。結(jié)論 巴曲酶注射液聯(lián)合地塞米松磷酸鈉注射液治療突發(fā)性耳聾具有較好的臨床療效，可改善凝血指標和血流動力學指標，安全性較好，具有一定的臨床推廣應用價值。

Objective To investigate the effect of Batroxobin Injection combined with Dexamethasone Sodium Phosphate Injection in treatment of sudden deafness. Methods Patients (60 cases) with sudden deafness in the Second Affiliated Hospital of Nanjing Medical University from June 2014 to July 2015 were randomly divided into control and treatment groups, and each group had 30 cases. Patients in the control group were intratympanic injection administered with Dexamethasone Sodium Phosphate Injection, 5 mg/time, three times weekly. Patients in the treatment group were iv administered with Batroxobin Injection on the basis of the control group, 10 BU added into normal saline 100 mL, every other day, review coagulation index since the second day, if FIB > 1.0, dosage halved. Patients in two groups were treated for 7 d. After treatment, the clinical efficacies were evaluated, and the clinical efficacies of deafness of different degrees, coagulation function indexes, hemodynamic indexes in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 70.0% and 83.3%, respectively, and there was difference between two groups (P<0.05). After treatment, the clinical efficacies of deafness of moderate, moderate to severe, and severe in the treatment group were significantly higher than those in the control group, and there was difference between two groups (P<0.05). After treatment, TT, PT, and ATPP in two groups were significantly increased, but the FIB in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, PV, HS, MS, and LS in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Batroxobin Injection combined with Dexamethasone Sodium Phosphate Injection has clinical curative effect in treatment of sudden deafness, can improve coagulation function index and hemodynamic index, with good safety, which has a certain clinical application value.
