[摘要]
目的 探討脈絡(luò)寧注射液聯(lián)合腦苷肌肽注射液治療急性缺血性腦血管病的臨床療效。方法 選取2015年3月-2017年3月在上海市松江區(qū)九亭醫(yī)院進(jìn)行治療的缺血性腦血管病患者142例為研究對(duì)象,隨機(jī)將患者分為對(duì)照組和治療組,每組各71例。對(duì)照組靜脈滴注腦苷肌肽注射液,20 mL加入到0.9%氯化鈉注射液250 mL中,1次/d;治療組在對(duì)照組基礎(chǔ)上靜脈滴注脈絡(luò)寧注射液,20 mL加入到0.9%氯化鈉注射液500 mL中,1次/d。兩組患者均治療3周。觀察兩組的臨床療效,比較兩組的美國(guó)國(guó)立衛(wèi)生研究院卒中量表(NIHSS)評(píng)分、改良的Rankin量表(mRS)評(píng)分、血清學(xué)指標(biāo)和血液流變學(xué)指標(biāo)。結(jié)果 治療后,對(duì)照組和治療組的總有效率分別為80.28%、95.77%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組NIHSS評(píng)分和mRS評(píng)分均顯著降低,同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組這些觀察指標(biāo)明顯低于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組血清超敏C反應(yīng)蛋白(hs-CRP)、可溶性血管細(xì)胞間黏附分子-1(sVCAM-1)、白細(xì)胞介素-6(IL-6)、單核細(xì)胞趨化蛋白(MCP-1)水平均顯著降低,胰島素樣生長(zhǎng)因子-1(IGF-1)水平顯著升高,同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組這些觀察指標(biāo)的改善程度明顯優(yōu)于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組血漿黏度(PV)、全血黏度高切(HS)、紅細(xì)胞聚集指數(shù)(EAI)、纖維蛋白原(FIB)水平均顯著降低,同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組這些觀察指標(biāo)明顯低于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 脈絡(luò)寧注射液聯(lián)合腦苷肌肽注射液治療急性缺血性腦血管病具有較好的臨床療效,可有效地改善患者神經(jīng)功能和血液流變學(xué)指標(biāo),降低機(jī)體炎癥反應(yīng),提高患者生活質(zhì)量,具有一定的臨床推廣應(yīng)用價(jià)值。

[Abstract]
Objective To investigate the clinical effect of Mailuoning Injection combined with Cattle Encephalon Glycoside and Ignotin Injection in treatment of acute ischemic cerebrovascular disease. Methods Patients (142 cases) with acute ischemic cerebrovascular disease in Songjiang District Jiuting Hospital of Shanghai from March 2015 to March 2017 were randomly divided into control and treatment groups, and each group had 71 cases. Patients in the control group were iv administered with Cattle Encephalon Glycoside and Ignotin Injection, 20 mL added into normal saline 250 mL, once daily. Patients in the treatment group were iv administered with Mailuoning Injection on the basis of the control group, 20 mL added into normal saline 500 mL, once daily. Patients in two groups were treated for 3 weeks. After treatment, the clinical efficacies were evaluated, NIHSS scores, mRS scores, serological indexes, and hemorheological indexes in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 80.28% and 95.77%, respectively, and there was difference between two groups (P<0.05). After treatment, NIHSS score and mRS score in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of hs-CRP, sICAM-1, IL-6, and MCP-1 in two groups were significantly decreased, but the levels of IGF-1 in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of PV, HS, FIB, and EAI in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Mailuoning Injection combined with Cattle Encephalon Glycoside and Ignotin Injection has clinical curative effect in treatment of acute ischemic cerebrovascular disease, can significantly improve neurological function and hemorheological indexes, decrease the body inflammatory reaction, and improve the quality of life, which has a certain clinical application value.
