目的 探究銀杏達(dá)莫注射液聯(lián)合坎地沙坦酯治療早期糖尿病腎病的臨床療效。方法 選取2015年5月-2016年12月在廈門市第五醫(yī)院進(jìn)行治療的236例早期糖尿病腎病，隨機(jī)分為對(duì)照組和治療組，每組各118例。對(duì)照組口服坎地沙坦酯片，1片/次，1次/d。治療組在對(duì)照組治療基礎(chǔ)上靜脈滴注銀杏達(dá)莫注射液，20 mL銀杏達(dá)莫注射液加入到250 mL生理鹽水，1次/d。兩組均連續(xù)治療8周。觀察兩組的臨床療效，同時(shí)觀察兩組治療前后血肌酐（Scr）、尿素氮（BUN）、24 h尿蛋白（TP/24 h）、總膽固醇（TC）、血流動(dòng)力學(xué)指標(biāo)的變化情況。結(jié)果 治療后，對(duì)照組和治療組的總有效率分別為83.05%、91.53%，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者TP/24 h、Scr、BUN、TC、血液黏度、纖維蛋白原均顯著降低，同組治療前后差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；治療后，治療組TP/24 h、Scr、BUN、TC、血液黏度、紅細(xì)胞壓積、纖維蛋白原低于對(duì)照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 銀杏達(dá)莫注射液聯(lián)合坎地沙坦酯治療糖尿病腎病具有較好的臨床療效，可降低患者尿微量蛋白水平，降低血液黏度，改善腎功能，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the therapeutic effect of Ginkgo Leaf Extract and Dipyridamole Injection combined with candesartan cilexetil in treatment of early diabetic nephropathy. Methods Patients (236 cases) with early diabetic nephropathy in Xiamen Fifth Hospital from May 2015 to December 2016 were randomly divided into control (118 cases) and treatment (118 cases) groups. Patients in the control group were po administered with Candesartan Cilexetil Tablets, 1 tablet/time, once daily. Patients in the treatment group were iv administered with Ginkgo Leaf Extract and Dipyridamole Injection on the basis of the control group, 20 mL Ginkgo Leaf Extract and Dipyridamole Injection added into 250 mL physiological saline, once daily. Patients in two groups were treated for 8 weeks. After treatment, the clinical efficacy was evaluated, and the changes of Scr, BUN, TP/24 h, TC, and hemodynamic indicators in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 83.05% and 91.53%, respectively, and there was difference between two groups (P<0.05). After treatment, TP/24 h, Scr, BUN, TC, blood viscosity, fibrinogen in two groups were decreased, and the difference was statistically significant in the same group (P<0.05). After treatment, Scr, BUN, TP/24 h, TC, blood viscosity, erythrocyte compression, and fibrinogen in the treatment group were lower than those in the control group, and there was difference between two groups (P<0.05). Conclusion Ginkgo Leaf Extract and Dipyridamole Injection combined with candesartan cilexetil has clinical curative effect in treatment of early diabetic nephropathy, and can reduce the urinary microprotein and blood viscosity, also can improve kidney function, which has a certain clinical application value.
