目的 探討谷紅注射液聯(lián)合丁苯酞注射液治療急性腦梗死的臨床療效。方法 選取2016年1月—2017年6月亳州市人民醫(yī)院收治的急性腦梗死患者136例，隨機(jī)分成對(duì)照組（68例）和治療組（68例）。對(duì)照組靜脈滴注丁苯酞氯化鈉注射液，100 mL/次，2次/d，兩次給藥間隔>6 h。治療組在對(duì)照組的基礎(chǔ)上靜脈滴注谷紅注射液，20 mL加入生理鹽水250 mL，1次/d。兩組患者均連續(xù)治療14 d。觀察兩組患者臨床療效，比較治療前后兩組患者美國(guó)國(guó)立衛(wèi)生研究院卒中量表（NIHSS）和Barthel指數(shù)、血液流變學(xué)指標(biāo)、以及血清腫瘤壞死因子-α（TNF-α）和丙二醛（MDA）水平。結(jié)果 治療后，對(duì)照組和治療組臨床總有效率分別為82.35%、94.12%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者NIHSS評(píng)分明顯降低，Barthel評(píng)分顯著升高，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組NIHSS評(píng)分和Barthel指數(shù)均顯著優(yōu)于對(duì)照組（P<0.05）。治療后，兩組患者全血高切黏度、全血低切黏度、血小板聚集率、血細(xì)胞比容均顯著降低（P<0.05）；且治療組各項(xiàng)血液流變學(xué)指標(biāo)水平比對(duì)照組更低（P<0.05）。治療后，兩組血清MDA水平均顯著降低，治療組患者血清TNF-α水平明顯降低，同組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組血清TNF-α和MDA水平明顯低于對(duì)照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 谷紅注射液聯(lián)合丁苯酞注射液治療急性腦梗死能有效改善患者機(jī)體血液流變學(xué)與微循環(huán)狀態(tài)，減輕神經(jīng)功能缺損。

Objective To evaluate the clinical efficacy of Guhong Injection combined with butylphthalide in treatment of acute cerebral infarction. Methods Patients (136 cases) with acute cerebral infarction in Bozhou People's Hospital from January 2016 to June 2017 were randomly divided into control (68 cases) and treatment (68 cases) groups. Patients in the control group were iv administered with Butylphthalide and Sodium Chloride Injection, 100 mL/time, twice daily, and the interval time using medicine was more than 6 h. Patients in the treatment group were iv administered with Guhong Injection on the basis of the control group, 20 mL added into normal saline 250 mL, once daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacy was evaluated, and the NIHSS and Barthel index scores, hemorheology indicators, serum TNF-α and MDA levels in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 82.35% and 94.12%, respectively, and there were differences between two groups (P < 0.05).After treatment, the NIHSS score in two groups was significantly decreased, but the Barthel score was significantly increased, and the difference was statistically significant in the same group (P < 0.05). And NIHSS and Barthel scores in the treatment group was better than those in the control group (P < 0.05). After treatment, the whole blood high viscosity, whole blood low viscosity, platelet aggregation rate, and hematocrit in two groups were significantly decreased, and there were differences in the same group (P < 0.05). And these hemorheology indicators in the treatment group were significantly lower than those in the control group (P < 0.05). After treatment, the MDA level in two groups was significantly decreased, but TNF-α level in the treatment group was significantly decreased, and there were differences in the same group (P < 0.05). After treatment, The TNF-α and MDA levels in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Guhong Injection combined with butylphthalide can effectively improve the hemorheology and microcirculation in treatment of acute cerebral infarction, and reduce the neurologic impairment.
