目的 探討保胎靈膠囊聯(lián)合間苯三酚治療先兆早產(chǎn)的臨床療效。方法 選取2016年9月—2017年9月在湖北民族學(xué)院附屬民大醫(yī)院治療的先兆早產(chǎn)患者76例，隨機分為對照組（38例）和治療組（38例）。對照組靜脈滴注間苯三酚注射液，120 mg加入5%葡萄糖注射液500 mL，滴速為30～40滴/min，依據(jù)宮縮情況調(diào)整速度至宮縮消失。治療組在對照組的基礎(chǔ)上口服保胎靈膠囊，1.5 g/次，3次/d，連續(xù)服用7 d。觀察兩組患者臨床療效，比較治療前后兩組患者相關(guān)臨床指標(biāo)、血清細(xì)胞因子水平和宮頸分泌物水平。結(jié)果 治療后，對照組和治療組的總有效率分別為78.95%、94.74%，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，治療組藥物起效時間和宮縮緩解時間明顯短于對照組，且妊娠延長時間明顯長于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組患者血清前列腺素E₂（PGE₂）、促腎上腺皮質(zhì)激素釋放激素（CRH）和NO水平顯著降低，孕酮（PROG）水平顯著增高，同組比較差異具有統(tǒng)計學(xué)意義（P<0.05）；且治療組上述血清細(xì)胞因子水平改善水平明顯優(yōu)于對照組（P<0.05）。治療后，兩組胎兒纖連蛋白（FFN）、磷酸化胰島素樣生長因子-1（IGFBP-1）和基質(zhì)金屬蛋白酶-9（MMP-9）水平均明顯降低（P<0.05）；且治療組上述宮頸分泌物量明顯低于對照組（P<0.05）。結(jié)論 保胎靈聯(lián)合間苯三酚治療先兆早產(chǎn)效果明顯，可有效延長妊娠時間，具有一定的臨床推廣應(yīng)用價值。

Objective To investigate the clinical curative effect of Baotailing Capsules combined with phloroglucinol in treatment of threatened premature labor. Methods Patients (76 cases) with threatened premature labor in University Hospital of Hubei University for Nationalities from September 2016 to September 2017 were randomly divided into control (38 cases) and treatment (38 cases) groups. Patients in the control group were iv administered with Phloroglucinol Injection, 120 mg added into 5% Glucose Injection 500 mL, the dripping speed was 30-40 drops/min, adjust the speed based on uterine contraction until uterine contraction disappeared. Patients in the treatment group were po administered with Baotailing Capsules on the basis of the control group, 1.5 g/time, three times daily, and they were treated for 7 d. After treatment, the clinical efficacy was evaluated, and the related clinical indicators, serum cytokine levels, and cervical secretions in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 78.95% and 94.74%, respectively, and there were differences between two groups (P < 0.05). After treatment, the time of drug onset and relieving uterine contraction in treatment group was shorter than that in the control group, but the prolongation time of pregnancy was significantly longer than that in the control group, with significant difference between two groups (P < 0.05). After treatment, the PGE₂, CRH, and NO levels in two groups were significantly decreased, PROG levels were significantly increased, and the difference was statistically significant in the same group (P < 0.05). And the improvement level of these serum cytokine in the treatment group was significantly better than that the control group (P < 0.05). After treatment, FFN, IGFBP-1, and MMP-9 levels in two groups were significantly decreased (P < 0.05). And the level of cervical secretions in the treatment group was significantly lower than that the control group (P < 0.05). Conclusion Baotailing Capsules combined with phloroglucinol has obvious effect in treatment of threatened premature labor, can effectively prolong the pregnancy time, which has a certain clinical application value.
