目的 探討卡泊三醇軟膏聯(lián)合轉(zhuǎn)移因子注射液治療扁平苔癬的臨床療效。方法 選取2014年6月—2016年8月在鄂東醫(yī)療集團(tuán)黃石市中心醫(yī)院治療的扁平苔癬患者90例為研究對象，所有患者按照隨機(jī)分組原則隨機(jī)分為對照組（40例）和治療組（50例）。對照組外用卡泊三醇軟膏，1 g/次，2次/d；同時口服硫酸奎寧片，0.3 g/次，2次/d，1周后改為1次/d。治療組外用卡泊三醇軟膏，1 g/次，2次/d；同時肌肉注射轉(zhuǎn)移因子注射液，3 mg/次，1次/周。兩組均連續(xù)治療3個月。觀察兩組的臨床療效，比較兩組的淋巴細(xì)胞亞群、細(xì)胞因子水平和銀屑病面積與嚴(yán)重性指數(shù)（PASI）評分。結(jié)果 治療后，對照組和治療組的總有效率分別為77.50%、96.00%，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組CD⁴⁺、CD⁴⁺/CD⁸⁺水平顯著升高，CD⁸⁺水平顯著降低，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)的改善程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組白細(xì)胞介素-18（IL-18）、白細(xì)胞介素-6（IL-6）、高敏C反應(yīng)蛋白（hs-CRP）水平均顯著降低，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)明顯低于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組PASI評分均顯著降低，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)明顯低于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 卡泊三醇軟膏聯(lián)合轉(zhuǎn)移因子注射液治療扁平苔癬具有較好的臨床療效，可改善臨床癥狀和免疫功能，調(diào)節(jié)細(xì)胞因子水平，安全性較好，具有一定的臨床推廣應(yīng)用價值。

Objective To investigate the clinical effect of Calcipotriol Ointment combined with Transfer Factor Injection in treatment of lichen planus. Methods Patients (90 cases) with lichen planus in Huangshi Central Hospital of Edong Medical Group from June 2014 to August 2016 were randomly divided into the control group (40 cases) and the treatment group (50 cases). Patients in the control group were topical administered with Calcipotriol Ointment, 1 g/time, twice daily. And patients in the control group were po administered with Quinine sulfate Tablets, 0.3 g/time, twice daily, after 1 week, changed to once daily. Patients in the treatment group were topical administered with Calcipotriol Ointment, 1 g/time, twice daily. And patients in the treatment group were intramuscular injection administered with Transfer Factor Injection, 3 mg/time, once weekly. Patients in two groups were treated for 3 months. After treatment, the clinical efficacies were evaluated, and lymphocyte subsets, cytokine levels, and PASI scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 77.50% and 96.00%, respectively, and there was difference between two groups (P < 0.05). After treatment, the levels of CD⁴⁺ and CD⁴⁺/CD⁸⁺ in two groups were significantly increased, but the levels of CD⁸⁺ in tow groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of IL-18, IL-6, and hs-CRP in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the PASI scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Calcipotriol Ointment combined with Transfer Factor Injection has clinical curative effect in treatment of lichen planus, can improve clinical symptoms and immune function, and regulate cytokine level, with good safety, which has a certain clinical application value.
