目的 探討鴉膽子油乳注射液聯(lián)合絲裂霉素和氟尿嘧啶治療肝癌的臨床療效。方法 選取2012年1月—2014年7月在咸陽(yáng)市第一人民醫(yī)院治療的肝癌患者102例，根據(jù)用藥的差別分為對(duì)照組（51例）和治療組（51例）。對(duì)照組靜脈注射注射用絲裂霉素，4 mg加入250 mL生理鹽水，1次/周；對(duì)照組患者同時(shí)靜脈注射氟尿嘧啶注射液，500 mg/m²加入250 mL生理鹽水，1次/d，第1～14天給藥；治療組在對(duì)照組的基礎(chǔ)上靜脈注射鴉膽子油乳注射液，20 mL加入250 mL生理鹽水，1次/d，第1～14天給藥。3周為1個(gè)療程，兩組均治療2個(gè)療程。觀察兩組患者臨床療效，比較治療前后兩組患者血清學(xué)指標(biāo)、QLQ-C30評(píng)分和生存率差別。結(jié)果 治療后，對(duì)照組客觀緩解率（ORR）和臨床獲益率（CBR）分別為54.90%、76.47%，均顯著低于治療組的70.59%、94.12%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組血清血管內(nèi)皮生長(zhǎng)因子（VEGF）、缺氧誘導(dǎo)因子-1A（HIF-1A）、結(jié)締組織生長(zhǎng)因子（CTGF）、基質(zhì)金屬蛋白酶-2（MMP-2）水平均明顯降低（P<0.05）；且治療組比對(duì)照組降低更顯著（P<0.05）。治療后，兩組患者軀體、認(rèn)知、情緒、角色和社會(huì)功能評(píng)分均明顯升高（P<0.05）；且治療組比對(duì)照組升高更明顯（P<0.05）。治療組1、2、3年生存率分別為88.24%、78.43%、64.71%，均分別高于同時(shí)期對(duì)照組患者（62.75%、56.86%、41.18%），兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 鴉膽子油乳注射液聯(lián)合絲裂霉素和氟尿嘧啶治療肝癌可有效改善患者生活質(zhì)量，提高生存率，有效抑制腫瘤新生血管生成，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical effect of Yadanzi Oil Emulsion Injection combined with mitomycin and fluorouracil in treatment of liver cancer. Methods Patients (102 cases) with liver cancer in the First People's Hospital of Xianyang City from January 2012 to July 2014 were randomly divided into control (51 cases) and treatment (51 cases) groups. Patients in the control group were iv administered with Mitomycin for injection, 4 mg added into normal saline 250 mL, once weekly. At the same time, they were iv administered with Fluorouracil Injection, 500 mg/m² added into normal saline 250 mL, once daily for the first two weeks. Patients in the treatment group were iv administered with Yadanzi Oil Emulsion Injection on the basis of the control group, 20 mg added into normal saline 250 mL, once daily for the first two weeks. Patients in two groups were treated for 2 courses, and a course was 3 weeks. After treatment, the clinical efficacy was evaluated, and the differences of serological indexes, QLQ-C30 scores and survival rate in two groups before and after treatment were compared. Results After treatment, the ORR and CBR in the control group were 54.90% and 76.47%, which were significantly lower than 70.59% and 94.12% in the treatment group, respectively, and there were differences between two groups (P < 0.05). After treatment, the VEGF, HIF-1A, CTGF, and MMP-2 levels in two groups were significantly decreased (P < 0.05). And the serological indexes levels in the treatment group were significantly lower than those in the control group (P < 0.05). After treatment, the physical, cognition, mood, role and social function scores in two groups were significantly increased (P < 0.05). And the QLQ-C30 scores in the treatment group were significantly higher than those in the control group (P < 0.05). The 1, 2, and 3 years survival rates in the treatment group were respectively 88.24%, 78.43% and 64.71%, which were significantly higher than 62.75%, 56.86%, and 41.18% in the control group, respectively, and there were differences between two groups (P < 0.05). Conclusion Yadanzi Oil Emulsion Injection combined with mitomycin and fluorouracil can effectively improve the quality of life and the survival rate in treatment of liver cancer, and inhibit tumor angiogenesis, which has a certain clinical application value.
