目的 探討銀杏蜜環(huán)口服溶液聯(lián)合甲鈷胺注射液治療突發(fā)性耳聾的臨床療效。方法 選取2016年8月—2017年8月在河北省滄州中西醫(yī)結(jié)合醫(yī)院進行治療的突發(fā)性耳聾患者86例為研究對象，根據(jù)用藥的差別將入組患者隨機分為對照組和治療組，每組各43例。對照組靜脈滴注甲鈷胺注射液，1 mg加入到0.9%氯化鈉注射液100 mL中，1次/d。治療組在對照組基礎(chǔ)上口服銀杏蜜環(huán)口服溶液，10 mL/次，3次/d。兩組患者均治療2周。觀察兩組的臨床療效，比較兩組的臨床癥狀改善時間、血液流變學(xué)和血清學(xué)指標(biāo)。結(jié)果 治療后，對照組和治療組的總有效率分別為81.40%、97.67%，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，治療組耳鳴、耳悶和眩暈恢復(fù)時間均短于對照組，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組血漿黏度（PV）、全血黏度高切（HS）、紅細(xì)胞聚集指數(shù)（EAI）水平均明顯降低，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)明顯低于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組血清可溶性血管細(xì)胞間黏附分子-1（sVCAM-1）、過氧化脂質(zhì)（LPO）、血栓素B₂（TXB₂）水平均明顯降低，降鈣素基因相關(guān)肽（CGRP）、超氧化物歧化酶（SOD）水平均明顯升高，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)的改善程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 銀杏蜜環(huán)口服溶液聯(lián)合甲鈷胺注射液治療突發(fā)性耳聾具有較好的臨床療效，可改善臨床癥狀，調(diào)節(jié)血液流變學(xué)指標(biāo)，修復(fù)血管內(nèi)皮損傷，改善機體氧化應(yīng)激水平，具有一定的臨床推廣應(yīng)用價值。

Objective To investigate the clinical effect of Ginkgo Leaf Extract and Armillariella Mellea Powders Oral Solution combined with Mecobalamin Injection in treatment of sudden deafness. Methods Patients (86 cases) with sudden deafness in Cangzhou Hospital of Integrated Traditional and Western Medicine of Hebei from August 2016 to August 2017 were enrolled in this study. According to the difference treatment plan, patients were divided into control and treatment groups, and each group had 43 cases. Patients in the control group were iv administered with Mecobalamin Injection, 1 mg added into normal saline 100 mL, once daily. Patients in the treatment group were po administered with Ginkgo Leaf Extract and Armillariella Mellea Powders Oral Solution on the basis of the control group, 10 mL/time, three times daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacies were evaluated, and clinical symptom improvement time, hemorheological indexes, and serological indexes in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 81.40% and 97.67%, respectively, and there was difference between two groups (P < 0.05). After treatment, the recovery time of tinnitus, ear muffle, and vertigo in the treatment group were shorter than those in the control group, and there was difference between two groups (P < 0.05). After treatment, the PV, HS, and EAI in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of sVCAM-1, LPO, and TXB₂ in two groups were significantly decreased, but the levels of CGRP and SOD in two groups were significantly increased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Ginkgo Leaf Extract and Armillariella Mellea Powders Oral Solution combined with Mecobalamin Injection has clinical curative effect in treatment of sudden deafness, can improve clinical symptoms, regulate hemorheological indexes, repair vascular endothelial injury, and improve the level of oxidative stress, which has a certain clinical application value.
