目的 探討心脈通膠囊聯(lián)合鹽酸川芎嗪治療不穩(wěn)定型心絞痛的臨床療效。方法 選取2013年8月—2016年8月在信陽市第一人民醫(yī)院進行治療的不穩(wěn)定型心絞痛患者178例，根據(jù)用藥差別分為對照組（89例）和治療組（89例）。對照組靜脈滴注注射用鹽酸川芎嗪，80 mg加入5%葡萄糖溶液250 mL，1次/d；治療組在對照組的基礎(chǔ)上口服心脈通膠囊，1.44 g/次，3次/d。兩組均經(jīng)過2周治療。觀察兩組患者臨床療效和心電圖療效，比較治療前后兩組患者心絞痛發(fā)作次數(shù)和持續(xù)時間，及血清學(xué)指標、血液流變學(xué)指標和血脂水平。結(jié)果 治療后，對照組患者臨床總有效率和心電圖總有效率分別為82.02%、79.78%，均分別顯著低于治療組的95.51%、94.38%，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組患者心絞痛發(fā)作次數(shù)、持續(xù)時間都顯著減少（P<0.05）；且治療組心絞痛發(fā)作次數(shù)和持續(xù)時間明顯優(yōu)于對照組（P<0.05）。治療后，兩組患者髓過氧化物酶（MPO）、白介素-1β（IL-1β）、基質(zhì)金屬蛋白酶-9（MMP-9）、妊娠相關(guān)血漿蛋白酶-A（PAPP-A）、血漿黏度（PV）、全血黏度高切（HS）、紅細胞聚集指數(shù)（EAI）和纖維蛋白原（FIB）水平，及膽固醇（TC）、三酰甘油（TG）和低密度脂蛋白膽固醇（LDL-C）血脂水平均顯著降低，同組治療前后比較差異具有統(tǒng)計學(xué)意義（P<0.05）；且治療組患者上述血清學(xué)指標、血液流變學(xué)指標和血脂水平均明顯低于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 心脈通膠囊聯(lián)合鹽酸川芎嗪治療不穩(wěn)定型心絞痛可顯著減少心絞痛發(fā)作次數(shù)，降低機體炎癥反應(yīng)和血脂水平。

Objective To investigate the clinical efficacy of Xinmaitong Capsules combined with Ligustrazine Hydrochloride for injection in treatment of unstable angina. Methods Patients (178 cases) with unstable angina in the First people's Hospital of Xinyang City from August 2013 to August 2016 were divided into control (89 cases) and treatment (89 cases) groups according to different treatments. Patients in the control group were iv administered with Ligustrazine Hydrochloride for injection, 80 mg added into 5% glucose solution 250 mL, once daily. Patients in the treatment group were po administered with Xinmaitong Capsules on the basis of the control group, 1.44 g/time, three times daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacy and ECG efficacy were evaluated, and the frequency and duration of angina pectoris, serological indexes, hemorheological indexes and blood lipid levels in two groups before and after treatment were compared. Results After treatment, the clinical and ECG efficacy in the control group were 82.02% and 79.78%, which were significantly lower than 95.51% and 94.38% in the treatment group, respectively, and there were differences between two groups (P < 0.05). After treatment, the frequency and duration of angina pectoris in two groups were significantly decreased (P < 0.05). And the improvement of frequency and duration of angina pectoris in the treatment group was significantly better than that in the control group (P < 0.05). After treatment, the MPO, IL-1β, MMP-9, PAPP-A, PV, HS, EAI, FIB, TC, TG and LDL-C levels in two groups were significantly decreased, and there were differences in the same group (P < 0.05). And the serological indexes, hemorheological indexes and blood lipid levels in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Xinmaitong Capsules combined with Ligustrazine Hydrochloride for injection in treatment of unstable angina can significantly reduce the frequency of angina pectoris, the inflammatory reaction, and blood lipid level.
