目的 探討西格列汀二甲雙胍片（Ⅱ）治療2型糖尿病的臨床療效。方法 選取2016年9月—2017年4月綿陽市中醫(yī)醫(yī)院收治的70例2型糖尿病患者為研究對象，隨機分為對照組和治療組，每組各35例。對照組患者口服鹽酸二甲雙胍片，0.5 g/次，3次/d。治療組患者口服西格列汀二甲雙胍片（Ⅱ），1片/次，2次/d。兩組患者均連續(xù)治療12周。觀察兩組的臨床療效，比較兩組的血糖指標、血脂指標、血清脂聯(lián)素（ADP）、銜接蛋白酶活化因子1（APAF1）、胱抑素C（CysC）水平和淋巴細胞亞群比較。結果 治療后，對照組和治療組的總有效率分別為85.71%、94.29%，兩組比較差異有統(tǒng)計學意義（P<0.05）。治療后，兩組患者的空腹血糖（FPG）、餐后2 h血糖（2 h PBG）、糖化血紅蛋白（HbA1c）均較治療前明顯降低，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療后治療組血糖指標明顯低于對照組，兩組比較差異有統(tǒng)計學意義（P<0.05）。兩組患者的TC、TG、LDL-C水平均較治療前明顯降低，HDL-C顯著升高，同組治療前后比較差異有統(tǒng)計學意義（P<0.05）；且治療后治療組的血脂指標優(yōu)于對照組，兩組比較差異有統(tǒng)計學意義（P<0.05）。治療后，兩組患者血清ADP、APAF1、CysC水平均較治療前明顯降低，同組治療前后比較差異具有統(tǒng)計學意義（P<0.05）；且治療后治療組患者ADP、APAF1、CysC水平明顯低于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組患者CD⁴⁺、CD⁴⁺/CD⁸⁺水平降低，CD⁸⁺水平升高，同組治療前后比較差異具有統(tǒng)計學意義（P<0.05）；且治療后治療組CD⁴⁺、CD⁸⁺和CD⁴⁺/CD⁸⁺水平明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。結論 磷酸西格列汀二甲雙胍（Ⅱ）治療2型糖尿病具有較好的臨床療效，可改善血糖、血脂指標，具有一定的臨床推廣應用價值。

Objective To investigate the clinical effect of Sitagliptin Metformin Tablets (Ⅱ) in treatment of type 2 diabetes. Methods Patients (70 cases) with type 2 diabetes in Mianyang Hospital of TCM from September 2016 to April 2017 were randomly divided into control group and treatment group, and each group had 35 cases. Patients in the control group were po administered with Metformin Hydrochloride Tablets, 0.5 g/time, three times daily. Patients in the treatment group were po administered with Sitagliptin Metformin Tablets (Ⅱ), 1 tablet/time, twice daily. Patients in two groups were treated for 12 weeks. After treatment, the clinical efficacies were evaluated, and blood sugar indexes, blood lipid indexes, ADP, APAF1, CysC levels, and lymphocyte subsets in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 85.71% and 94.29%, respectively, and there was difference between two groups (P < 0.05). After treatment, FPG, 2 h PBG, and HbA1c in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the blood sugar indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). TC, TG, and LDL-C in two groups were significantly decreased, but HDL-C was significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the blood lipid indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of ADP, APAF1, and CysC in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And those indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). The levels of CD⁴⁺ and CD⁴⁺/CD⁸⁺ in two groups were significantly increased, but the level of CD⁸⁺ in two groups was significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Sitagliptin Metformin Tablets (Ⅱ) has good effect in treatment of type 2 diabetes, and can improve blood sugarand blood lipid indexes, which has a certain clinical application value.
