目的 探討五苓散聯(lián)合徒手淋巴引流術(shù)治療下肢創(chuàng)傷后急性腫脹的臨床療效。方法 選取2014年6月—2017年6月廣東省工傷康復(fù)中心收治的下肢創(chuàng)傷后急性腫脹患者100例，隨機(jī)分為對照組和治療組，每組各50例。對照組給予徒手淋巴引流，治療組在對照組治療基礎(chǔ)上口服五苓散，150 mL/次，午晚飯后1 h各服1次。兩組均連續(xù)治療1周。觀察兩組的臨床療效，比較兩組治療前后血漿黏度（ηb）、血小板聚集率（PAR）、健患肢周徑差、疼痛視覺模擬評分法（VAS）評分的變化情況。結(jié)果 治療后，對照組和治療組患者的總有效率分別為82%、98%，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組ηb、PAR、健患肢周徑差、VAS評分均明顯低于治療前，同組治療前后差異有統(tǒng)計學(xué)意義（P<0.05）；治療后，治療組ηb、PAR、健患肢周徑差、VAS評分低于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 五苓散聯(lián)合徒手淋巴引流術(shù)治療下肢創(chuàng)傷后急性腫脹具有較好的臨床療效，有利于緩解患者疼痛及腫脹癥狀，具有一定的臨床推廣應(yīng)用價值。

Objective To investigate the therapeutic effect of Wuling Powder combined with manual lymphatic drainage in treatment of acute swelling after lower limb trauma. Methods Patients (100 cases) with acute swelling after lower limb trauma in Guangdong Work Injury Rehabilitation Hospital from June 2014 to June 2017 were randomly divided into control (50 cases) and treatment (50 cases) groups. Patients in the control group were given manual lymphatic drainage treatment. Patients in the treatment group were po administered with Wuling Powder, 150 mL/time, 1 h after lunch and dinner. Patients in two groups were treated for 1 week. After treatment, the clinical efficacy was evaluated, and the changes of ηb, PAR, circumference difference of healthy and sick limb, and VAS score in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 82% and 98%, respectively, and there was difference between two groups (P < 0.05). After treatment, ηb, PAR, circumference difference of healthy and sick limb, and VAS score in two groups were decreased, and the difference was statistically significant in the same group (P < 0.05). After treatment, ηb, PAR, circumference difference of healthy and sick limb, and VAS score in the treatment group were lower than those in the control group, and there was difference between two groups (P < 0.05). Conclusion Wuling Powder combined with manual lymphatic drainage has clinical curative effect in treatment of acute swelling after lower limb trauma, and can relieve the pain and swelling of the patients, which has a certain clinical application value.
