目的 探討百令膠囊聯(lián)合注射用細(xì)辛腦治療慢性阻塞性肺疾病急性加重期（AECOPD）的臨床效果。方法 選擇瀘州市人民醫(yī)院2016年4月—2017年5月接收的AECOPD患者116例，隨機(jī)分為對(duì)照組（58例）和治療組（58例）。對(duì)照組靜脈滴注注射用細(xì)辛腦，24 mg加入5%葡萄糖注射液250 mL，1次/d。治療組在對(duì)照組的基礎(chǔ)上口服百令膠囊，5粒/次，3次/d。兩組患者連續(xù)治療14 d。觀察兩組患者臨床療效，比較治療前后兩組患者呼吸困難評(píng)分（MMRC）、輔助呼吸肌動(dòng)用評(píng)分、血?dú)夥治龊头喂δ堋?b>結(jié)果 治療后，對(duì)照組患者臨床有效率為82.76%，顯著低于治療組的94.83%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P < 0.05）。治療后，兩組患者M(jìn)MRC評(píng)分和輔助呼吸肌動(dòng)用評(píng)分均顯著低于治療前（P < 0.05）；且治療組上述評(píng)分比對(duì)照組降低的更明顯（P < 0.05）。治療后，兩組患者動(dòng)脈血氧分壓（pO₂）、血氧飽和度（SaO₂）均顯著高于治療前，動(dòng)脈血二氧化碳分壓（pCO₂）顯著降低，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P < 0.05）；且治療組血?dú)夥治鲋笜?biāo)改善更明顯（P < 0.05）。治療后，兩組患者用力肺活量（FVC）、第1秒用力呼氣量（FEV1）、FEV1占預(yù)計(jì)值百分比（FEV1% pred）、FEV1/FVC均顯著高于治療前（P < 0.05）；且治療組患者肺功能顯著優(yōu)于對(duì)照組（P < 0.05）。結(jié)論 百令膠囊聯(lián)合注射用細(xì)辛腦治療AECOPD療效顯著，可明顯改善血?dú)夥治龊头喂δ?，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To evaluate the clinical effect of Bailing Capsules combined with Asarone for injection in treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods Patients (116 cases) with AECOPD in Luzhou People's Hospital from April 2016 to May 2017 were randomly divided into control (58 cases) and treatment (58 cases) groups. Patients in the control group were iv administered with Asarone for injection, 24 mg added into 5% glucose injection 250 mL, once daily. Patients in the treatment group were po administered with Bailing Capsules on the basis of the control group, 5 grains/time, three times daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacy was evaluated, and the MMRC scores, the activity of accessory respiratory muscles scores, blood gas analysis and pulmonary function in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 82.76%, which was significantly lower than 94.83% in the treatment group, and there were differences between two groups (P < 0.05). After treatment, the MMRC scores and the activity of accessory respiratory muscles scores in two groups were significantly decreased (P < 0.05), and these scores in the treatment group were significantly lower than those in the control group (P < 0.05). After treatment, the pO₂ and SaO₂ in two groups were significantly increased, but pCO₂ was significantly decreased, and there were differences in the same group (P < 0.05). And the blood gas indexes in the treatment group were significantly better than those in the control group (P < 0.05). After treatment, the FVC, FEV1, FEV1% pred and FEV1/FVC in two groups were significantly increased (P < 0.05), and the pulmonary function in the treatment group were significantly better than that in the control group (P < 0.05). Conclusion Bailing Capsules combined with Asarone for injection has significant effect in treatment of AECOPD, and can significantly improve blood gas analysis and lung function, which has a certain clinical application value.
