[摘要]
目的 觀察參芎葡萄糖注射液聯(lián)合腦苷肌肽治療新生兒缺氧缺血性腦病的臨床療效。方法 收集2015年1月—2017年4月鄂州市中心醫(yī)院新生兒科收治的新生兒缺氧缺血性腦病患兒67例,隨機(jī)分為對(duì)照組(30例)和治療組(37例),對(duì)照組患兒靜脈滴注腦苷肌肽注射液,2 mL加入到0.9%氯化鈉注射液50 mL中,1次/d。治療組患兒在對(duì)照組的基礎(chǔ)上靜脈滴注參芎葡萄糖注射液,50 mL/次,1次/d。兩組患兒均治療14 d。觀察兩組患者臨床療效,比較治療前后兩組患者臨床體征恢復(fù)情況、血清一氧化氮(NO)、內(nèi)皮素-1(ET-1)、脂聯(lián)素(APN)和胰島素樣生長(zhǎng)因子-1(IGF-1)、神經(jīng)行為評(píng)分(NBNA)和預(yù)后情況。結(jié)果 治療后,對(duì)照組和治療組臨床總有效率分別為73.33%、83.78%,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P < 0.05)。治療組中、重度患兒意識(shí)障礙恢復(fù)時(shí)間、肌張力恢復(fù)時(shí)間均短于對(duì)照組,輕、中、重度患兒原始反射恢復(fù)時(shí)間均短于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P < 0.05)。治療后,兩組患兒血清NO、APN和IGF-1水平均升高,ET-1水平降低,同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義(P < 0.05);且治療后治療組上述指標(biāo)均顯著優(yōu)于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P < 0.05)。兩組NBNA評(píng)分均顯著升高,同組治療前后比較差異具有統(tǒng)計(jì)學(xué)意義(P < 0.05);且治療后治療組NBNA評(píng)分顯著高于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P < 0.05)。回訪時(shí),治療組患兒MDI和PDI評(píng)分均顯著高于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P < 0.05)。結(jié)論 參芎葡萄糖注射液聯(lián)合腦苷肌肽治療新生兒缺氧缺血性腦病臨床效果顯著,可有效改善患兒臨床癥狀及減少后遺癥的發(fā)生,并可有效改善血清指標(biāo),具有一定的臨床推廣應(yīng)用價(jià)值。

[Abstract]
Objective To observe the clinical effect of Shenxiong Glucose Injection combined with cattle encephalon glycoside and ignotin in treatment of hypoxic ischemic encephalopathy of newborn. Methods Patients (67 cases) with hypoxic ischemic encephalopathy of newborn in Department of Neonatology in Ezhou Central Hospital from January 2015 to April 2017 were randomly divided into control (30 cases) and treatment (37 cases) groups. Patients in the control group were iv administered with Cattle Encephalon Glycoside and Ignotin Injection, 2 mL added into normal saline 50 mL, once daily. Patients in the treatment group were iv administered with Shenxiong Glucose Injection, 50 mL/time, once daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacy was evaluated, recovery of clinical signs, NO, ET-1, APN, IGF-1, NBNA, and conditions of prognosis in two groups were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 73.33% and 83.78%, respectively, and there was difference between two groups (P < 0.05). Consciousness recovery time and muscle tension recovery time of moderate and server children in the treatment group were shorter than those in the control group, with significant difference between two groups (P < 0.05). After treatment, NO, APN, and IGF-1 levels were increased, while ET-1 level was decreased, and the difference was statistically significant in the same group (P < 0.05). And those indexes in the treatment group were better than those in the control group, with significant difference between two groups (P < 0.05). After treatment, NBNA score in two groups were increased, and the difference was statistically significant in the same group (P < 0.05). And NBNA score in the treatment group was higher than that in the control group, with significant difference between two groups (P < 0.05). When the children were followed up, MDI and PDI scores in the treatment group were higher than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Shenxiong Glucose Injection combined with cattle encephalon glycoside and ignotin has a good clinical efficacy in treatment of hypoxic ischemic encephalopathy of newborn, can significantly improve the clinical symptoms and prognosis, and can improve serum indexes, which has a certain clinical application value.
