目的 探討十味龍膽花顆粒聯(lián)合頭孢曲松鈉治療老年社區(qū)獲得性肺炎的臨床療效。方法 選取2016年8月-2017年8月在商丘市第五人民醫(yī)院進(jìn)行治療的老年社區(qū)獲得性肺炎患者92例，根據(jù)用藥差別分為對照組（46例）和治療組（46例）。對照組患者靜脈滴注注射用頭孢曲松鈉，2 g加入生理鹽水100 mL，2次/d；治療組在對照組的基礎(chǔ)上口服十味龍膽花顆粒，3 g/次，3次/d。兩組患者均經(jīng)過7 d治療。觀察兩組患者臨床療效，比較治療前后兩組患者臨床癥狀改善時間和血清學(xué)指標(biāo)差異。結(jié)果 治療后，對照組和治療組臨床有效率分別為80.43%、95.65%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，治療組在發(fā)熱、咳痰和肺部啰音消失時間方面均顯著短于對照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組血清C反應(yīng)蛋白（CRP）、N端前腦鈉肽（NT-proBNP）、腫瘤壞死因子-α（TNF-α）和降鈣素（PCT）水平均顯著降低，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組血清學(xué)指標(biāo)顯著低于對照組，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 十味龍膽花顆粒聯(lián)合注射用頭孢曲松鈉治療老年社區(qū)獲得性肺炎療效顯著，可有效改善臨床癥狀，降低機(jī)體炎癥反應(yīng)，具有一定的臨床推廣應(yīng)用價值。

Objective To investigate the clinical efficacy of Shiwei Longdanhua Granules combined with ceftriaxone sodium in treatment of senile community acquired pneumonia. Methods Patients (92 cases) with community acquired pneumonia in Shangqiu Fifth People's Hospital from August 2016 to August 2017 were divided into control (46 cases) and treatment (46 cases) groups according to different treatments. Patients in the control group were iv administered with Ceftriaxone Sodium for injection, 2 g added into normal saline 100 mL, twice daily. Patients in the treatment group were po administered with Shiwei Longdanhua Granules on the basis of the control group, 3 g/time, three times daily. Patients in two groups were treated for 7 d. After treatment, the clinical efficacy was evaluated, and the differences of the improvement time of clinical symptom, the serological indexes in two groups before and after treatment were compared.Results After treatment, the clinical efficacy in the control and treatment groups were 80.43% and 95.65%, respectively, and there were differences between two groups (P < 0.05). After treatment, the disappearance time of fever, expectoration, and lung rale in the treatment group was significantly shorter than that in the control group, with significant difference between two groups (P < 0.05). After treatment, the CRP, NT-proBNP, TNF-α, and PCT levels in two groups were significantly decreased, and there were differences in the same group (P < 0.05), and these serological indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Shiwei Longdanhua Granules combined with ceftriaxone sodium has good effect in treatment of senile community acquired pneumonia can effectively improve the clinical symptoms and reduce the inflammatory reaction, which has a certain clinical application value.
