目的 探討槐杞黃顆粒聯(lián)合布地奈德氣霧劑治療兒童咳嗽變異性哮喘的臨床療效。方法 選取2016年1月-2017年1月吉林大學第一醫(yī)院兒科收治的咳嗽變異性哮喘患兒106例進行回顧性分析，根據治療方法的不同將患兒分為對照組和治療組，每組各53例。對照組吸入布地奈德氣霧劑，<2歲：100 μg/次，≥ 2歲：200 μg/次，2次/d。治療組在對照組基礎上口服槐杞黃顆粒，2~3歲：5 g/次，3~12歲：10 g/次，2次/d。兩組患兒均連續(xù)治療3個月。觀察兩組的臨床療效，比較兩組的肺功能指標、免疫功能指標、血清免疫球蛋白E（IgE）和細胞因子水平。結果 治療后，對照組和治療組的總有效率分別為83.0%、90.6%，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組肺活量（FVC）、第一秒呼出氣體容積（FEV1）、FEV1/FVC均顯著升高，同組治療前后比較差異具有統(tǒng)計學意義（P<0.05）；且治療組這些肺功能指標明顯高于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組CD⁴⁺、CD⁴⁺/CD⁸⁺水平均顯著下降，CD⁸⁺水平顯著升高，同組治療前后比較差異具有統(tǒng)計學意義（P<0.05）；且治療組這些觀察指標的改善程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。治療后，兩組γ-干擾素（IFN-γ）水平顯著升高，IgE、白細胞介素-4（IL-4）水平均顯著下降，同組治療前后比較差異具有統(tǒng)計學意義（P<0.05）；且治療組這些觀察指標的改善程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學意義（P<0.05）。結論 槐杞黃顆粒聯(lián)合布地奈德氣霧劑治療兒童咳嗽變異性哮喘具有較好的臨床療效，可改善患兒肺功能，調節(jié)免疫功能，降低復發(fā)率，具有一定的臨床推廣應用價值。

Objective To study the clinical efficacy of Huaiqihuang Granules combined with Budesonide Aerosol in treatment of cough variant asthma in children. Methods Children (106 cases) with cough variant asthma in the First Hospital of Jilin University from January 2016 to January 2017 were enrolled in this study. According to the difference treatment plan, patients were randomly divided into control and treatment groups, and each group had 53 cases. Children in the control group were inhalation administered with Budesonide Aerosol, ages < 2 years:100 μg/time, ages ≥ 2 years:200 μg/time, twice daily. Children in the treatment group were po administered with Huaiqihuang Granules on the basis of the control group, 2 years ≤ ages < 3 years:5 g/time, 3 years ≤ ages < 12 years:10 g/time, twice daily. Children in two groups were treated for 3 months. After treatment, the clinical efficacies were evaluated, and pulmonary function indexes, immune function indexes, serum IgE, and cytokine levels in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 83.0% and 90.6%, respectively, and there was difference between two groups (P < 0.05). After treatment, FVC, FEV1, and FEV1/FVC in two groups were significantly increased, and the difference was statistically significant in the same group (P < 0.05). And the pulmonary function indexes in the treatment group were significantly higher than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of CD⁴⁺ and CD⁴⁺/CD⁸⁺ in two groups were significantly decreased, but the levels of CD⁸⁺ in two groups were significantly increased, and the difference was statistically significant in the same group (P < 0.05). And the immune function indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of IFN-γ in two groups were significantly increased, but the levels of IgE and IL-4 in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Huaiqihuang Granules combined with Budesonide Aerosol has clinical curative effect in treatment of cough variant asthma in children, can improve pulmonary function, regulate immune function, and decrease recurrence rate, which has a certain clinical application value.
