目的 探討應(yīng)用雙歧桿菌三聯(lián)活菌膠囊聯(lián)合血必凈注射液治療術(shù)后早期炎性腸梗阻的臨床效果。方法 選取宜昌市第二人民醫(yī)院2015年3月—2017年3月收治的術(shù)后早期炎性腸梗阻患者96例，隨機(jī)分為對照組（48例）和治療組（48例）。對照組靜脈滴注血必凈注射液，50 mL加入生理鹽水100 mL，2次/d。治療組在對照組基礎(chǔ)上口服雙歧桿菌三聯(lián)活菌膠囊，2粒/次，2次/d。兩組均連續(xù)治療1周。觀察兩組患者臨床療效，比較治療前后兩組患者胃腸減壓量、癥狀改善時(shí)間、炎性因子、淋巴細(xì)胞和PCT水平及氧化應(yīng)激和腸黏膜功能指標(biāo)變化。結(jié)果 治療后，對照組和治療組的臨床有效率分別為85.42%和95.83%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，治療組胃腸減壓量明顯低于對照組（P<0.01），且癥狀改善時(shí)間均明顯短于對照組（P<0.01）。治療后，兩組腹腔引流液中C反應(yīng)蛋白（CRP）、腫瘤壞死因子（TNF）-α、白細(xì)胞介素（IL）-6和IL-1β水平顯著下降（P<0.01）；且治療組比對照組降低更顯著（P<0.01）。治療后，兩組患者外周血CD⁴⁺和CD⁴⁺/CD⁸⁺比值均顯著增加，外周血CD¹⁹⁺值和血清降鈣素原（PCT）水平顯著降低，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.01）；且治療組患者淋巴細(xì)胞和PCT水平比對照組改善更明顯（P<0.01）。治療后，兩組患者血清超氧化物歧化酶（SOD）水平顯著升高，血清丙二醛（MDA）、二胺氧化酶（DAO）、D-乳酸水平顯著降低，同組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.01）；且治療組上述指標(biāo)比對照組改善更顯著（P<0.01）。結(jié)論 雙歧桿菌三聯(lián)活菌膠囊聯(lián)合血必凈注射液治療術(shù)后早期炎性腸梗阻可迅速緩解患者腹痛、腹脹等癥狀，有效減輕腸壁粘膜炎癥，改善腸黏膜功能。

Objective To investigate the clinical effect of Bifidobacterium Triple Live Capsules combined with Xuebijing Injection in treatment of early postoperative inflammatory ileus. Methods Patients (96 cases) with early postoperative inflammatory ileus in the Second People's Hospital of Yichang from March 2015 to March 2017 were randomly divided into control (48 cases) and treatment (48 cases). Patients in the control group were iv administered with Xuebijing Injection, 50 mL added into normal saline 100 mL, twice daily. Patients in the treatment group were po administered with Bifidobacterium Triple Live Capsules on the basis of the control group, 2 grains/time, twice daily. Patients in two groups were treated for 1 week. After treatment, the clinical efficacy was evaluated, and the changes of the gastrointestinal decompression, the symptom improvement time, the inflammatory factors, the lymphocytes and PCT levels, the oxidative stress and intestinal mucosal function in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 85.42% and 95.83% respectively, and there were differences between two groups (P < 0.05). After treatment, the gastrointestinal decompression in the treatment group was significantly lower than that in the control group (P < 0.01), the symptom improvement time the treatment group was significantly shorter than that in the control group (P < 0.01). After treatment, the CRP, TNF-α, IL-6 and IL-1β levels in two groups were significantly decreased (P < 0.01). And the inflammatory factors levels in the treatment group after treatment were significantly lower than those in the control group (P < 0.01). After treatment, the CD⁴⁺ and CD⁴⁺/CD⁸⁺ of peripheral blood in two groups were significantly increased, but the CD¹⁹⁺ of peripheral blood and PCT levels were significantly decreased, and there were differences in the same group (P < 0.01). And the lymphocytes and PCT levels in the treatment group after treatment were significantly better than those in the control group, with significant difference between two groups (P < 0.01). After treatment, the SOD levels in two groups were significantly increased, but the MDA, DAO and D-lactate levels were significantly decreased, and the difference was statistically significant in the same group (P < 0.01). And these indicators in the treatment group after treatment were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Bifidobacterium Triple Live Capsules combined with Xuebijing Injection in treatment of early postoperative inflammatory ileus can quickly alleviate the symptoms of abdominal pain and abdominal distension, and effectively reduce the intestinal mucosal inflammation, and improve the intestinal mucosal function.
