[摘要]
目的 探討骨愈靈片聯(lián)合鮭降鈣素注射液治療老年骨質疏松癥胸腰椎壓縮性骨折的臨床療效。方法 選取2016年7月-2017年7月在鄭州市骨科醫(yī)院治療的骨質疏松性胸腰椎壓縮骨折患者76例為研究對象,采用計算機隨機分組法將患者分為對照組和治療組,每組各38例。對照組肌肉注射鮭魚降鈣素注射液,第1天50 IU/次,之后100 IU/次,1次/d,連續(xù)治療20 d,而后停藥20 d,之后重復上述方式。治療組在對照組治療基礎上口服骨愈靈片,2 g/次,3次/d。兩組均連續(xù)治療4個月。觀察兩組的臨床療效,比較兩組的骨密度(BMD)、血清骨轉換指標水平、視覺模擬評分法(VAS)評分和Oswestry功能障礙指數(shù)問卷表(ODI)評分。結果 治療后,對照組和治療組的總有效率分別為81.58%、97.37%,兩組比較差異有統(tǒng)計學意義(P<0.05)。治療后,兩組血清β-膠原降解產物(β-CTX)、I型前膠原氨基端前肽(PINP)、骨鈣素(BGP)、骨堿性磷酸酶(BALP)水平顯著降低,BMD、25羥維生素D[25-(OH) D]水平顯著升高,同組治療前后比較差異有統(tǒng)計學意義(P<0.05);且治療組這些觀察指標明顯優(yōu)于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。治療后,兩組VAS評分和ODI評分均顯著降低,同組治療前后比較差異有統(tǒng)計學意義(P<0.05);且治療組這些評分明顯低于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。結論 骨愈靈片聯(lián)合鮭降鈣素注射液治療老年骨質疏松癥胸腰椎壓縮性骨折具有較好的臨床療效,可緩解疼痛,加快骨折愈合,增加骨密度,調節(jié)血清骨轉換指標,具有一定的臨床推廣應用價值。

Objective To explore the clinical effect of Guyuling Tablets combined with Salmon Calcitonin Injection in treatment of senile osteoporosis complicated with compression fracture of thoracolumbar vertebrae. Methods Elderly patients (76 cases) with osteoporosis complicated with compression fracture of thoracolumbar vertebrae in Zhengzhou Orthopedics Hospital from July 2016 to July 2017 were divided into control and treatment groups according to computer random grouping method, and each group had 38 cases. Patients in the control group were im administered with Salmon Calcitonin Injection, first day 50 IU/time, afterwards 100 IU/time, once daily, treated for 20 d, then stopped for 20 d, and repeated the above method. Patients in the treatment group were po administered with Guyuling Tablets on the basis of the control group, 2 g/time, three times daily. Patients in two groups were treated for 4 months. After treatment, the clinical efficacies were evaluated, and BMD, serum bone conversion indexes, VAS scores, and ODI scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 81.58% and 97.37%, respectively, and there was difference between two groups (P<0.05). After treatment, the levels of β-CTX, PINP, BGP, and BALP in two groups were significantly decreased, but the levels of BMD and 25-(OH)D in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, VAS scores and ODI scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the scores in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Guyuling Tablets combined with Salmon Calcitonin Injection has clinical curative effect in treatment of senile osteoporosis complicated with compression fracture of thoracolumbar vertebrae, can relieve patient's pain, accelerate patient's fracture healing, increase bone mineral density, and regulate serum bone turnover indexes, which has a certain clinical application value.
