目的 探討寒喘祖帕顆粒聯(lián)合注射用甲潑尼龍琥珀酸鈉治療兒童頑固喘憋性肺炎的臨床效果。方法 選取2016年4月-2017年6月深圳市龍崗中心醫(yī)院兒科收治的82例頑固喘憋性肺炎患兒，根據(jù)隨機分組法將所有患兒分為對照組和治療組，每組各41例。對照組靜脈滴注注射用甲潑尼龍琥珀酸鈉，1.5 mg/kg，3次/d。治療組在對照組治療的基礎(chǔ)上口服寒喘祖帕顆粒，2 g/次，2次/d。兩組患兒均連續(xù)治療6 d。比較兩組患兒的臨床療效，同時比較治療前后兩組患兒的臨床癥狀改善情況、血氣指標和炎性指標。結(jié)果 治療后，對照組和治療組的總有效率分別為75.6%、92.7%，兩組總有效率比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療組氣喘緩解時間、咳嗽消失時間、喘鳴音消失時間和平均住院時間均顯著短于對照組，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組氧分壓（pO₂）明顯升高，二氧化碳分壓（pCO₂）明顯降低，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組血氣指標明顯優(yōu)于對照組，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組血清TNF-α、IL-13、IL-1β水平均明顯下降，同組治療前后比較差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組炎性指標水平明顯低于對照組，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 寒喘祖帕顆粒聯(lián)合注射用甲潑尼龍琥珀酸鈉治療兒童頑固喘憋性肺炎具有較好臨床療效，可有效改善臨床癥狀和血氣指標，降低炎癥狀態(tài)，具有一定的臨床推廣應(yīng)用價值。

Objective To investigate the clinical effect of Hanchuanpazu Granules combined with Methylprednisolone Sodium Succinate for injection in treatment of persistent asthmatic pneumonia in children. Methods Children (82 cases) with persistent asthmatic pneumonia in Shenzhen Longgang Central Hospital from April 2016 to June 2017 were randomly divided into control group (41 cases) and treatment group (41 cases). Children in the control group were iv administered with Methylprednisolone Sodium Succinate for injection, 1.5 mg/kg, 3 times daily. Children in the treatment group were po administered with Hanchuanpazu Granules on the basis of the control group, 2 g/time, twice daily. Patients in two groups were treated for 6 d. After treatment, the clinical efficacies were evaluated, and clinical symptom improvement, blood gas index, and inflammatory index in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 75.6% and 92.7%, respectively, and there was difference between two groups (P<0.05). After treatment, the remission time of asthma, the time of cough disappearance, the time of wheezing disappearance, and the average hospital stay in the treatment group were significantly shorter than those in the control group, with significant difference between two groups (P<0.05). After treatment, pO₂ in two groups were significantly increased, but pCO₂ in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the blood gas indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of TNF-α, IL-13, and IL-1β in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the inflammatory indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Hanchuanpazu Granules combined with Methylprednisolone Sodium Succinate for injection has clinical curative effect in treatment of persistent asthmatic pneumonia in children, can improve clinical symptom and blood gas index, decrease the levels of inflammatory factor, which has a certain clinical application value.
