目的 探討復(fù)方氯己定含漱液聯(lián)合替硝唑治療牙周病的臨床療效。方法 選取2017年5月-2017年11月在梧州市人民醫(yī)院治療的牙周病患者102例，隨機(jī)分為對照組（51例）和治療組（51例）。對照組口服替硝唑片，0.5 g/次，2次/d；治療組在對照組基礎(chǔ)上早晚含漱復(fù)方氯己定含漱液，15 mL/次。兩組經(jīng)過4周治療。觀察兩組患者臨床療效，同時(shí)比較治療前后兩組患者牙周附著指數(shù)、牙菌斑指數(shù)、牙齦指數(shù)、牙齒松動(dòng)度、炎癥指標(biāo)、OHIP-14和VAS評分。結(jié)果 治療后，對照組臨床有效率為80.39%，顯著低于治療組的96.08%，兩組比較差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者牙周附著指數(shù)、牙菌斑指數(shù)、牙齦指數(shù)和牙齒松動(dòng)度均顯著降低（P<0.05），且治療組降低程度明顯優(yōu)于對照組（P<0.05）。治療后，兩組血清白細(xì)胞介素-6（IL-6）、IL-Iβ、腫瘤壞死因子-α（TNF-α）、基質(zhì)金屬蛋白酶3（MMP-3）水平均顯著降低（P<0.05），且治療后治療組IL-Iβ、IL-6、TNF-α和MMP-3水平明顯低于對照組（P<0.05）。治療后，兩組OHIP-14和VAS評分均顯著降低（P<0.05），且治療后治療組OHIP-14和VAS評分明顯低于對照組（P<0.05）。結(jié)論 復(fù)方氯己定含漱液聯(lián)合替硝唑治療牙周病可有效降低機(jī)體炎癥水平，改善患者口腔健康狀況，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To investigate the clinical efficacy of Compound Gargle Solution Chlorhexidine Giuconatie combined with tinidazole in treatment of periodontal diseases. Methods Patients (102 cases) with periodontal diseases in the People's Hospital of Wuzhou from May 2017 to November 2017 were randomly divided into control (51 cases) and treatment (51 cases) groups. Patients in the control group were po administered with Tinidazole Tablete, 0.5 g/time, twice daily. Patients in the treatment group were gargling administered with Compound Gargle Solution Chlorhexidine Giuconatie on the basis of the control group daily morning and evening, 4 grains/time. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacy was evaluated, and the periodontal attachment index, dental plaque index, gingival index, tooth mobility, inflammatory markers, OHIP-14 and VAS in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 80.39%, which was significantly lower than 96.08% in the treatment group, and there were differences between two groups (P<0.05). After treatment, the periodontal attachment index, dental plaque index, gingival index, tooth mobility in two groups were significantly decreased (P<0.05), and these indexes in the treatment group after treatment were significantly lower than those in the control group (P<0.05). After treatment, the IL-Iβ, IL-6, TNF-α and MMP-3 levels in two groups were significantly decreased (P<0.05), and these inflammatory markers levels in the treatment group after treatment were significantly lower than those in the control group (P<0.05). After treatment, the OHIP-14 and VAS in two groups were significantly decreased (P<0.05), and the OHIP-14 and VAS in the treatment group after treatment were significantly lower than that in the control group (P<0.05). Conclusion Compound Gargle Solution Chlorhexidine Giuconatie combined with tinidazole in treatment of periodontal diseases can effectively reduce the inflammation and improve oral health, which has a certain clinical application value.
