目的 回顧性調(diào)查河南省人民醫(yī)院住院患者使用注射用紅花黃色素的超說明書現(xiàn)狀，為臨床合理用藥提供參考。方法 隨機(jī)選取河南省人民醫(yī)院2018年1~12月使用注射用紅花黃色素的住院患者醫(yī)囑1 420例。參照藥品說明書，對適應(yīng)癥、用法用量、用藥療程、配伍溶媒、聯(lián)合用藥等合理性進(jìn)行分析。結(jié)果 1 420例醫(yī)囑中，1 293例存在不同程度的超說明書用藥情況，超說明書用藥發(fā)生率為91.06%，表現(xiàn)為超適應(yīng)癥（61.76%）、超單次劑量（63.66%）、超用藥頻次（0.28%）、超療程（4.44%）、超溶媒量（22.39%）、配伍（0.42%）和溶媒選擇不當(dāng)（12.82%）。結(jié)論 注射用紅花黃色素的超說明書用藥情況廣泛存在，建議盡快出臺(tái)中藥注射劑的相關(guān)政策，科學(xué)化管理，并加強(qiáng)監(jiān)測與管控，促進(jìn)臨床合理用藥。

Objective To investigate the current status of off label medication of Safflor Yellow for injection for inpatients in Henan Provincal People's Hospital in 2018, and provide a reference for clinical rational drug use. Methods 1 420 Orders of inpatients who used Safflor Yellow for injection from January to December 2018 were selected. According to the drug instructions, the rationality of indications, dosage, medication course, compatible solvents, and combined medication were analyzed. Results There were 1 293 off label medication orders in 1 420 cases of prescription, and the incidence rate of off label medication was 91.06%, including over indication (61.76%), over single dose (63.66%), over frequency (0.28%), over course (4.44%), over solvent amounts (22.39%), compatibility (0.42%), and improper solvent(12.82%). Conclusion Safflor Yellow for injection is widely off label used in Henan Provincal People's Hospital. It is suggested that policies for traditional Chinese medicine injection should be issued as soon as possible, monitoring and control management should be strengthened to promote rational use of drugs.
