目的 研究銀杏酮酯滴丸聯(lián)合阿加曲班注射液治療急性腦梗死的臨床療效。方法 選取2017年8月-2019年8月安陽市第六人民醫(yī)院收治的106例急性腦梗死患者為研究對象，所有患者隨機(jī)分為對照組和治療組，每組各53例。對照組患者靜脈注射阿加曲班注射液，20 mg阿加曲班注射液溶于250 mL生理鹽水中，1次/d；治療組在對照組基礎(chǔ)上口服銀杏酮酯滴丸，8丸/次，3次/d。兩組患者持續(xù)治療14 d。觀察兩組的臨床療效，NIHSS評分、BI指數(shù)評分和mRS評分及高敏C反應(yīng)蛋白（hs-CRP）、白細(xì)胞介素-6（IL-6）、腫瘤壞死因子-α（TNF-α）、血栓素A₂（TXA₂）、內(nèi)皮素-1（ET-1）和一氧化氮（NO）水平。結(jié)果 治療后，對照組和治療組的總有效率分別為81.13%、96.23%，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組NIHSS評分和mRS評分明顯下降，BI指數(shù)評分明顯升高（P<0.05），且治療組各評分改善較多（P<0.05）。治療后，兩組hs-CRP、IL-6、TNF-α水平顯著降低（P<0.05）；并且治療組hs-CRP、IL-6、TNF-α水平降低較多（P<0.05）。治療后，兩組血清TXA₂、ET-1水平均明顯降低，NO水平明顯升高（P<0.05），且治療組TXA₂、ET-1、NO水平改善較多（P<0.05）。結(jié)論 銀杏酮酯滴丸聯(lián)合阿加曲班注射液治療急性腦梗死具有較好的治療效果，改善神經(jīng)功能和日常生活活動(dòng)能力，調(diào)節(jié)血清因子水平，安全性較高，值得在臨床上推廣應(yīng)用。

Objective To study the effect of Ginkgo Bilobate Dropping Pills combined with Argatroban Injection in treatment of acute cerebral infarction. Methods Patients (106 cases) with acute cerebral infarction who received treatment in Anyang Sixth People's Hospital from August 2016 to August 2019 were randomly divided into two groups, and each group had 53 cases. Patients in the control group were iv administered with Argatroban Injection, 20 mg added into normal saline 250 mL, once daily. Patients in the treatment group were po administered with Ginkgo Bilobate Dropping Pills on the basis of the control group, 8 pills/time, three times daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacies were evaluated, and NIHSS score, mRS score, BI index score, and the levels of hs-CRP, IL-6, TNF-α, TXA₂, ET-1, and NO in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 81.13% and 96.23%, respectively, and there was difference between two groups (P<0.05). After treatment, NIHSS score and mRS score in two groups were significantly decreased, but BI index score in two groups were significantly increased (P<0.05). And the NIHSS score, mRS score, and BI index score in the treatment group were significantly better than those in the control group (P<0.05). After treatment, the levels of hs-CRP, IL-6, and TNF-α in two groups were significantly decreased (P<0.05). And the levels of hs-CRP, IL-6, and TNF-α in the treatment group were significantly lower than those in the control group (P<0.05). After treatment, the levels of TNF-α and TXA₂ in two groups were significantly decreased, but the levels of NO in two groups were significantly increased (P<0.05). And the levels of TXA₂, ET-1, and NO in the treatment group were significantly better than those in the control group (P<0.05). Conclusion Ginkgo Bilobate Dropping Pills combined with Argatroban Injection has clinical curative effect in treatment of acute cerebral infarction, can improve nerve function and activity of daily life, regulate serum factor level, with good safety, and which is worthy of clinical application.
