目的 研究鴉膽子油軟膠囊聯(lián)合SOX方案（奧沙利鉑+替吉奧）治療晚期結直腸癌的臨床療效。方法 選取2018年8月-2019年8月在鄭州大學附屬洛陽中心醫(yī)院治療的80例晚期結直腸癌患者，所有患者隨機分為對照組和治療組，每組各40例。對照組患者入院后給予SOX方案治療，第1~14天給予注射用奧沙利鉑，推薦劑量130 mg/m²，溶于500 mL 5%葡萄糖溶液中，輸注時間小于3 h；第1~21天口服替吉奧膠囊，1粒/次，2次/d。治療組在對照組基礎上口服鴉膽子油軟膠囊，4粒/次，3次/d。21 d為1個治療療程，兩組患者接受治療3個療程。觀察兩組的臨床療效，比較Karnofsky（KPS）評分、生活質(zhì)量（QOL）評分及血清癌胚抗原（CEA）、基質(zhì)金屬蛋白酶（MMP）-2和MMP-9水平。結果 經(jīng)過治療后，治療組客觀緩解率（ORR）和疾病控制率（CBR）顯著高于對照組（P<0.05）。治療后，兩組患者KPS評分和QOL評分顯著升高（P<0.05），且治療組KPS評分和QOL評分升高程度較大（P<0.05）。治療后，兩組CEA、MMP-2和MMP-9水平顯著降低（P<0.05）；并且治療組降低較多（P<0.05）。治療組胃腸道反應、血小板減少、白細胞減少和轉氨酶升高等不良反應發(fā)生率明顯低于對照組（P<0.05）。結論 鴉膽子油軟膠囊聯(lián)合SOX方案治療晚期結直腸癌具有較好的治療效果，能夠提高患者生活質(zhì)量，降低腫瘤標志物水平，安全性較高，值得在臨床上推廣應用。

Objective To study the clinical efficacy of Yadanzi Oil Soft Capsules combined with SOX chemotherapy regimen (Oxaliplatin + Tegafur, Gimeracil and Oteracil Potassium) in treatment of advanced colorectal cancer. Methods Patients (80 cases) with advanced colorectal cancer in Luoyang Central Hospital Affiliated to Zhengzhou University from August 2018 to August 2019 were randomly divided into control and treatment groups, and each group had 40 cases. Patients in the control group were given SOX chemotherapy regimen, and patients were iv administered with Oxaliplatin for injection at 1-14 d, 130 mg/m² added into 5% glucose solution 500 mL, infusion time was less than 3 h; and at 1-14 d, patients were po administered with Gimeracil and Oteracil Porassium Capsules, 1 grain/time, twice daily. Patients in the treatment group were po administered with Yadanzi Oil Soft Capsules on the basis of the control group, 4 grains/time, three times daily. A course of treatment had 21 d, and patients in two groups were treated for 3 courses. After treatment, the clinical efficacies were evaluated, and KPS score, QOL score, the levels of CEA, MMP-2, and MMP-9 in two groups were compared. Results After treatment, ORR and CBR in the treatment group were significantly higher than those in the control group, and there were differences between two groups (P<0.05). After treatment, KPS score and QOL score in the two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05), and KPS score and QOL score in the treatment group was significantly higher than those in the control group (P<0.05). After treatment, the levels of CEA, MMP-2, and MMP-9 in the two groups were significantly decreased (P<0.05), and the serum tumor markers in the treatment group was significantly lower than those in the control group (P<0.05). The incidence of gastrointestinal reaction, thrombocytopenia, leucopenia, and transaminase elevation in the treatment group was significantly lower than that in the control group, and there was difference between two groups (P<0.05). Conclusion Yadanzi Oil Soft Capsules combined with SOX chemotherapy regimen has clinical curative effect in treatment of advanced colorectal cancer, can improve the quality of life of patients, reduce the level of tumor markers, with good safety, which is worthy of clinical application.
