[摘要]
目的 探討脈絡(luò)通顆粒聯(lián)合注射用尤瑞克林治療腦梗死的臨床療效。方法 選擇2018年12月—2020年1月在鄭州大學(xué)附屬鄭州中心醫(yī)院進(jìn)行治療的126例腦梗死患者,根據(jù)數(shù)字表法隨機(jī)分為對(duì)照組和治療組,每組各63例。對(duì)照組患者靜脈滴注注射用尤瑞克林,0.15 PNA溶于100 mL 0.9%氯化鈉注射液中,1次/d。治療組患者在對(duì)照組治療基礎(chǔ)上口服脈絡(luò)通顆粒,6 g/次,3次/d。7 d為1個(gè)療程,兩組患者均連續(xù)治療4個(gè)療程。觀察兩組的臨床療效,比較兩組的美國(guó)國(guó)立衛(wèi)生研究院卒中量表(NIHSS)評(píng)分、Barthel指數(shù)評(píng)定表(MBI)評(píng)分、腦血流量、腦血容量、血漿黏稠度、血清炎性因子水平。結(jié)果 治療后,治療組總有效率96.83%,對(duì)照組為85.71%,治療組顯著高于對(duì)照組(P<0.05)。治療后,兩組患者的NIHSS評(píng)分顯著降低,MBI評(píng)分顯著升高(P<0.05),治療組的改善程度顯著優(yōu)于對(duì)照組(P<0.05)。治療后,兩組患者腦血流量、腦血容量均顯著提高,血漿黏稠度顯著下降(P<0.05),治療組腦血流量、腦血容量顯著高于對(duì)照組,血漿黏稠度顯著低于對(duì)照組(P<0.05)。治療后,兩組患者血清核轉(zhuǎn)錄因子-κB(NF-κB)、腫瘤壞死因子(TNF-α)、超敏C反應(yīng)蛋白(hs-CRP)水平較治療前均顯著下降(P<0.05),治療組NF-κB、TNF-α、hs-CRP水平下降程度顯著大于對(duì)照組(P<0.05)。結(jié)論 脈絡(luò)通顆粒聯(lián)合注射用尤瑞克林治療腦梗死患者有較好的療效,可有效改善患者神經(jīng)功能、腦血流量、血漿黏稠度及血清中各項(xiàng)相關(guān)指標(biāo),促進(jìn)患者日?;顒?dòng)能力恢復(fù),具有一定臨床使用價(jià)值。

[Abstract]
Objective To explore the effects of Mailuotong Granules combined with Urinary Kallidinogenase for injection in treatment of cerebral infarction. Methods Patients (126 cases) with cerebral infarction in Zhengzhou Central Hospital Affiliated to Zhengzhou University from December 2018 to January 2020 were randomly divided into control and treatment groups, and each group had 63 cases. Patients in the control group were iv administered with Urinary Kallidinogenase for injection, 0.15 PNA added into 0.9% sodium chloride injection 100 mL, once daily. Patients in the treatment group were po administered with Mailuotong Granules on the basis of the control group, 6 g/time, three times daily. A course of treatment had 7 d, and patients in two groups were treated for 4 courses. After treatment, the clinical efficacies were evaluated, and NIHSS scores, MBI scores, cerebral blood volume flow, cerebral volume and plasma viscosity, and serum inflammatory factor levels in two groups were compared. Results After treatment, the total effective rate in the control and treatment groups was 96.83% and 85.71%, which was significantly higher in the treatment group than that in the control group (P<0.05). After treatment, NIHSS score in the two groups were significantly reduced, but MBI scores were significantly increased (P<0.05), and the improvement in the treatment group was significantly better than that in the control group (P<0.05). After treatment, the cerebral blood flow and cerebral blood volume in the two groups were significantly increased, but the plasma viscosity was significantly decreased (P<0.05). The cerebral blood flow and cerebral blood volume in the treatment group were significantly higher than those in the control group, but the plasma viscosity was significantly lower than that in the control group (P<0.05). After treatment, the levels of NF-κB, TNF-α, and hs-CRP in the two groups were significantly decreased (P<0.05), and the levels of NF-κB, TNF-α, and hs-CRP in the treatment group were significantly lower than those in the control group (P<0.05). Conclusion Mailuotong Granules combined with Urinary Kallidinogenase for injection has clinical curative effect in treatment of cerebral infarction, can effectively improve the patients' nerve function, cerebral blood flow, plasma viscosity and various related indexes in serum, and promote the recovery of patients' daily activity ability, which has a certain clinical value.
