目的 研究復(fù)方鮮竹瀝液聯(lián)合噻托溴銨粉霧劑治療慢性阻塞性肺疾病急性加重期的臨床療效。方法 選取2018年9月—2019年9月在安陽市第六人民醫(yī)院治療的100例慢性阻塞性肺疾病急性加重期患者，將所有患者隨機(jī)分為對(duì)照組和治療組，每組各50例。對(duì)照組吸入噻托溴銨粉霧劑，1粒/次，1次/d。治療組在對(duì)照組基礎(chǔ)上口服復(fù)方鮮竹瀝液，20 mL/次，3次/d。兩組持續(xù)治療3個(gè)月。觀察兩組的臨床療效，比較血?dú)庵笜?biāo)、肺功能指標(biāo)、血清炎性因子水平。結(jié)果 治療后，治療組總有效率94.00%顯著高于對(duì)照組82.00%（P<0.05）。治療后，兩組二氧化碳分壓（pCO₂）顯著降低，血氧分壓（pO₂）顯著升高（P<0.05），且治療組血?dú)庵笜?biāo)改善程度較大（P<0.05）。治療后，兩組第一秒用力呼氣容積（FEV1）占預(yù)計(jì)值百分比（FEV1%）、每分鐘最大通氣量（MVV）顯著升高，殘氣量/肺總量（RV/TLC）明顯降低（P<0.05）；且治療組肺功能指標(biāo)改善較多（P<0.05）。治療后，兩組白細(xì)胞介素-17（IL-17）、IL-8、腫瘤壞死因子-α（TNF-α）水平顯著降低（P<0.05）；并且治療組血清炎性因子水平降低較多（P<0.05）。結(jié)論 復(fù)方鮮竹瀝液聯(lián)合噻托溴銨粉霧劑治療慢性阻塞性肺疾病急性加重期具有較好的臨床療效，可改善血?dú)庵笜?biāo)和肺功能指標(biāo)，降低血清炎性因子水平，值得在臨床上推廣應(yīng)用。

Objective To study the clinical effect of Compound Xianzhuli Liquids combined with Tiotropium Bromide Powder for inhalation in treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods Patients (100 cases) with AECOPD in Anyang People's Hospital NO. 6 from September 2013 to September 2015 were randomly divided into control and treatment groups, and each group had 50 cases. Patients in the control group were inhalation administered with Tiotropium Bromide Powder for inhalation, 1 grain/time, once daily. Patients in the treatment group were po administered with Compound Xianzhuli Liquids on the basis of the control group, 20 mL/time, three times daily. Patients in two groups were treated for 3 months. After treatment, the clinical efficacies were evaluated, and blood gas indexes, pulmonary function indexes, and the serum levels of inflammatory factors in two groups were compared. Results After treatment, the total effective rate in the treatment group was 94.00%, which was significantly higher than that in the control group (82.00%) (P<0.05). After treatment, the level of pCO₂ in the two groups were significantly decreased, but the levels of pO₂ in two groups were significantly increased (P<0.05), and the blood gas indexes in the treatment group were better than those in the control group (P<0.05). After treatment, FEV1% and MVV in the two groups were significantly increased, but RV/TLC in the two groups were significantly decreased (P<0.05), and the pulmonary function indexes in the treatment group were better than those in the control group (P<0.05). After treatment, the levels of IL-17, IL-8, and TNF-α in the two groups were significantly decreased (P<0.05), and the the serum levels of inflammatory factors in the treatment group were better than those in the control group (P<0.05). Conclusion Compound Xianzhuli Liquids combined with Tiotropium Bromide Powder for inhalation has clinical curative effect in treatment of AECOPD, can improve blood gas index and lung function index, reduce serum level inflammatory factor, which is worthy of clinical application.
