目的 研究牙痛寧滴丸聯(lián)合米諾環(huán)素軟膏治療慢性牙周炎的臨床療效。方法 選取2017年10月—2019年10月在淅川縣中醫(yī)院治療的60例慢性牙周炎患者（110顆患牙），將所有患者隨機分為對照組和治療組，每組各30例55顆患牙。對照組患者給予鹽酸米諾環(huán)素軟膏，1次/周，連續(xù)用4次。治療組在對照組基礎(chǔ)上口服牙痛寧滴丸，10粒/次，3次/d。兩組患者治療4周。觀察兩組患者臨床療效，對比兩組患者治療前后牙周指標(biāo)、口腔保健自我效能量表（SESS）評分、口腔健康影響程度量表（OHIP-14）評分和視覺模擬評分法（VAS）評分、齦溝液炎性因子水平變化。結(jié)果 治療后，治療組總有效率（96.36%）顯著高于對照組（83.64%）（P<0.05）。治療后，兩組患者SESS評分顯著升高，OHIP-14和VAS評分均顯著降低（P<0.05）；且治療組SESS評分高于對照組，OHIP-14和VAS評分均低于對照組（P<0.05）。治療后，兩組患者菌斑指數(shù)（PLI）、牙周袋深度（PD）、齦溝出血指數(shù)（SBI）、牙周附著水平（AL）均顯著降低（P<0.05），且治療組牙周指標(biāo)降低程度較大（P<0.05）。治療后，兩組患者齦溝液腫瘤壞死因子-α（TNF-α）和白細(xì)胞介素-1β（IL-1β）水平均顯著降低（P<0.05），且治療組齦溝液炎性因子水平降低程度較大（P<0.05）。結(jié)論 牙痛寧滴丸聯(lián)合米諾環(huán)素軟膏治療慢性牙周炎具有較好的治療效果，能改善牙周指標(biāo)和臨床癥狀，降低齦溝液炎性因子水平，安全性較高，值得在臨床上推廣應(yīng)用。

Objective To study the effect of Yatongning Dropping Pills combined with Minocycline Hydrochloride Ointment in treatment of chronic periodontitis. Methods Patients (60 cases, 110 teeth) with chronic periodontitis in Xichuan County Hospital of Traditional Chinese Medicine from October 2017 to October 2019 were randomly divided into control and treatment groups, and each group had 42 cases (55 teeth). Patients in the control group were given Minocycline Hydrochloride Ointment, once weekly, continuously administered 4 times. Patients in the treatment group were po administered with Yatongning Dropping Pills on the basis of the control group, 10 grains/time, three times daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacies were evaluated, and periodontal indexes, SESS scores, OHIP-14 scores, VAS scores, and the levels of inflammatory factors in gingival crevicular fluid before and after treatment in two groups were compared. Results After treatment, the total effective rate of the treatment group (96.36%) was significantly higher than that of the control group (83.64%) (P<0.05). After treatment, the SESS scores of two groups were significantly increased, but the OHIP-14 scores and VAS scores of two groups were significantly decreased (P<0.05). And the SESS score in the treatment group was significantly higher than that in the control group, but the OHIP-14 score and VAS score in the control group were significantly lower than those in the control group (P<0.05). After treatment, the PLI, PD, SBI, and AL of the two groups were significantly decreased (P<0.05), and the periodontal indexes of the treatment group were significantly lower than those of the control group (P<0.05). After treatment, the levels of TNF-α and IL-1β in gingival crevicular fluid of two groups were significantly decreased (P<0.05), and the levels of inflammatory factors in gingival crevicular fluid of the treatment group were significantly lower than those of the control group (P<0.05). Conclusion Yatongning Dropping Pills combined with Minocycline Hydrochloride Ointment has clinical curative effect in treatment of chronic periodontitis, can improve periodontal indexes and clinical symptoms, reduce the levels of inflammatory factors in gingival crevicular fluid, with high safety, which is worthy of clinical application.
