[摘要]
目的 研究丙戊酸在兒童癲癇患者中的血藥濃度監(jiān)測(cè)的意義和臨床療效的影響因素,旨在幫助兒童患者獲得有效的血藥濃度和滿意的臨床有效率提供依據(jù)。方法 選取2019年6月-2020年3月監(jiān)測(cè)丙戊酸血藥濃度的106例15歲及以下的癲癇患者,對(duì)其血藥濃度監(jiān)測(cè)結(jié)果進(jìn)行分析,并對(duì)性別、年齡、每日單位體質(zhì)量給藥劑量、劑型和聯(lián)合用藥等因素對(duì)血藥濃度和臨床療效的影響進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果 給藥劑量與血藥濃度之間存在正相關(guān)性(P<0.01)。年齡和藥物劑型對(duì)血藥濃度的影響具有統(tǒng)計(jì)學(xué)意義(P<0.05);患兒年齡(3~6歲組和7~15歲組)、血藥濃度、每日單位體質(zhì)量給藥劑量、服用丙戊酸鈉緩釋片等對(duì)臨床療效的影響有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 丙戊酸血藥濃度監(jiān)測(cè)是評(píng)估和預(yù)測(cè)臨床療效的重要指標(biāo);血藥濃度影響因素復(fù)雜,給藥劑量與丙戊酸血藥濃度存在顯著的正相關(guān)性,年齡和藥物劑型對(duì)丙戊酸血藥濃度的影響具有臨床意義;年齡≥ 3歲、血藥濃度、給藥劑量和服用丙戊酸緩釋片是藥物臨床療效的獨(dú)立影響因素。

[Abstract]
Objective To study the significance of blood concentration monitoring of valproic acid in children with epilepsy and the influencing factors of clinical efficacy, in order to help children obtain effective blood concentration and satisfactory clinical effectiveness. Methods A total of 106 epilepsy patients aged 15 and under who monitored the blood concentration of valproic acid from June 2019 to March 2020 were selected. The monitoring results of their blood concentration were analyzed, and the gender, age, and dose (mg/(kg·d)), the dosage form and the combination of drugs and other factors have a statistical analysis on the influence of blood concentration and clinical efficacy. Results There is a positive correlation between the administered dose and the blood concentration (P<0.01). The effect of age and drug formulation on blood drug concentration is statistically significant (P<0.05). The effect of children's age (3-6 years old group and 7-15 years old group), blood drug concentration, dose, taking sodium valproate sustained-release tablets on the clinical efficacy is statistically significant (P<0.05). Conclusion The monitoring of blood concentration of valproic acid is an important indicator for evaluating and predicting clinical efficacy. The influencing factors of blood concentration are complicated. There is a significant positive correlation between the dose and the blood concentration of valproic acid. Age and dosage form have a significant effect on valproic acid. The influence of blood drug concentration is of clinical significance; age ≥ 3 years, blood drug concentration, dosage and taking valproic acid sustained-release tablets are independent factors influencing the clinical efficacy of drugs.
