目的 探討固本咳喘膠囊聯(lián)合噻托溴銨吸入粉霧劑治療慢性阻塞性肺疾病穩(wěn)定期的臨床療效。方法 選取2018年8月—2020年7月在河南科技大學(xué)第二附屬醫(yī)院就診的90例慢性阻塞性肺疾病患者作為研究對(duì)象，按照隨機(jī)數(shù)字表法將90例患者分為對(duì)照組和治療組，每組各45例。對(duì)照組霧化吸入噻托溴銨吸入粉霧劑，18 μg/次，1次/d。治療組在對(duì)照組基礎(chǔ)上口服固本咳喘膠囊，4粒/次，3次/d。兩組患者連續(xù)治療3個(gè)月。觀察兩組的臨床療效，并比較兩組的肺功能指標(biāo)、血清炎癥因子水平。結(jié)果 治療后，治療組的總有效率為95.56%，對(duì)照組的總有效率為82.22%，組間比較有明顯差異（P<0.05）。治療后，兩組的第1秒鐘用力呼吸容積（FEV₁）/用力肺活量（FVC）、最大呼氣流量（PEF）、第一秒用力呼氣容積占預(yù)計(jì)值百分比（FEV₁ pre）顯著升高（P<0.05）；且治療組的肺功能指標(biāo)比對(duì)照組高，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組的白細(xì)胞介素-17（IL-17）、腫瘤壞死因子-α（TNF-α）、和肽素（copeptin）水平顯著降低（P<0.05）；且治療后治療組的IL-17、TNF-α、copeptin水平比對(duì)照組低，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 固本咳喘膠囊聯(lián)合噻托溴銨吸入粉霧劑可提高慢性阻塞性肺疾病穩(wěn)定期的療效，改善肺功能，有助于降低炎癥因子的水平，且安全性良好。

Objective To investigate the clinical efficacy of Guben Kechuan Capsules combined with Tiotropium Bromide Powder for inhalation in treatment of stable chronic obstructive pulmonary disease. Methods Patients (90 cases) with stable chronic obstructive pulmonary disease in the Second Affiliated Hospital of Henan University of Science and Technology from August 2018 to July 2020 were randomly divided into control and treatment groups, and each group had 45 cases. Patients in the control group were atomization inhalation of Tiotropium Bromide Powder for inhalation, 18 μg/time, once daily. Patients in the treatment group were po administered with Guben Kechuan Capsules on the basis of the control group, 4 grains/time, three times daily. Patients in two groups were treated for 3 months. After treatment, the clinical efficacies were evaluated, and the lung function indexes and serum inflammatory factors of two groups were compared. Results After treatment, the total effective rate of the treatment group was 95.56%, and that of the control group was 82.22%, and there was a significant difference between the two groups (P<0.05). After treatment, the FEV₁/FVC, PEF, FEV₁ pre of two groups were significantly increased (P<0.05). And after treatment, the FEV₁/FVC, PEF, and FEV₁ pre of the treatment group were higher than those of the control group, and the difference was statistically significant (P<0.05). After treatment, the levels of IL-17, TNF-α, and copeptin in two groups were significantly decreased (P<0.05). After treatment, the levels of IL-17, TNF-α, and copeptin in the treatment group were lower than those in the control group, and the difference was statistically significant (P<0.05). Conclusion Guben Kechuan Capsules combined with Tiotropium Bromide Powder for inhalation can improve the curative effect of stable chronic obstructive pulmonary disease, improve lung function, help to reduce the level of inflammatory factors, with good safety.
