目的 探討肺寧顆粒聯(lián)合注射用頭孢唑肟鈉治療老年急性發(fā)作期慢性支氣管炎的臨床療效。方法 選擇2019年5月—2020年5月在平頂山市第二人民醫(yī)院治療的96例老年急性發(fā)作期慢性支氣管炎患者，根據(jù)就診順序分為對照組（48例）和治療組（48例）。對照組靜脈滴注注射用頭孢唑肟鈉，2 g加入生理鹽水100 mL，12 h/次；治療組在對照組的基礎(chǔ)上口服肺寧顆粒，5 g/次，3次/d。兩組均經(jīng)2周治療。觀察兩組患者臨床療效，比較治療前后兩組患者臨床癥狀改善時間，血清超敏C反應蛋白（hs-CRP）、巨噬細胞刺激蛋白（MSP）、降鈣素原（PCT）、白三烯B4（LTB4）、白細胞介素-8（IL-8）和巨噬細胞炎癥蛋白-1α（MIP-1α）水平，肺功能第1秒用力呼氣末容積（FEV1）、用力肺活量（FVC）和最大呼氣流速（PEF），及動脈血氣指標氧分壓（pO₂）和二氧化碳分壓（pCO₂）水平。結(jié)果 經(jīng)治療，對照組總有效率為81.25%，顯著低于治療組的97.92%（P<0.05）。經(jīng)治療，治療組臨床癥狀改善時間上均明顯早于對照組（P<0.05）。經(jīng)治療，兩組血清hs-CRP、MSP、PCT、LTB4、IL-8、MIP-1α水平均顯著降低，并以治療組降低最明顯（P<0.05）。經(jīng)治療，兩組FEV1、FVC、PEF、pO₂、pCO₂水平均明顯改善，并以治療組改善最顯著（P<0.05）。結(jié)論 老年急性發(fā)作期慢性支氣管炎患者采用肺寧顆粒聯(lián)合注射用頭孢唑肟鈉治療可明顯改善其癥狀，促進肺功能恢復，降低機體炎癥水平，具有一定的臨床推廣應用價值。

Objective To investigate the clinical effect of Feining Granules combined with ceftizoxime sodium in treatment of elderly patients with acute exacerbation of chronic bronchitis. Methods Patients (96 cases) with acute exacerbation of chronic bronchitis in Pingdingshan the Second People’s Hospital from May 2019 to May 2020 were divided into control (48 cases) and treatment (48 cases) groups based on visiting order. Patients in the control group were iv administered with Ceftizoxime Sodium for injection, 2 g added into 100 mL normal saline, 12 h/time. Patients in the treatment group were po administered with Feining Granules on the basis of the control group, 5 g/time, three times daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacy was evaluated, and the improvement time of clinical symptoms, and the serum levels of hs-CRP, MSP, PCT, LTB4, IL-8, and MIP-1α, the pulmonary function indexes of FEV1, FVC and PEF, and arterial blood gas indexes of pO₂ and pCO₂ in two groups before and after treatment were compared. Results After treatment, the effective rate in the control group was 81.25%, which was significantly lower than 97.92% in the treatment group (P<0.05). After treatment, the improvement time of clinical symptoms in the treatment group was significantly earlier than that in the control group (P<0.05). After treatment, the serum levels of hs-CRP, MSP, PCT, LTB4, IL-8, and MIP-1α in the two groups were decreased (P<0.05), and especially in the treatment group (P<0.05). After treatment, the levels of FEV1, FVC, PEF, pO₂, and pCO₂ were significantly improved in two groups (P<0.05), especially in the treatment group (P<0.05). Conclusion Feining Granules combined with ceftizoxime sodium in treatment of elderly patients with acute exacerbation of chronic bronchitis can effectively improve the clinical symptoms, which is conducive to the improvement of lung function and the decrease of serum inflammatory factors, which has a certain clinical application value.
