目的 探討復(fù)方玄駒膠囊聯(lián)合洛索洛芬鈉治療骨關(guān)節(jié)炎的臨床效果。方法 選取2019年12月—2020年11月天津中醫(yī)藥大學(xué)第一附屬醫(yī)院收治的126例骨關(guān)節(jié)炎患者，隨機分成對照組（63例）和治療組（63例）。對照組口服洛索洛芬鈉片，60 mg/次，3次/d。在對照組基礎(chǔ)上，治療組口服復(fù)方玄駒膠囊，1.26 g/次，3次/d。兩組患者連續(xù)治療4周。觀察兩組患者臨床療效，比較治療前后兩組患者典型臨床表現(xiàn)改善情況，西安大略和麥克馬斯特大學(xué)骨關(guān)節(jié)炎指數(shù)（WOMAC）和關(guān)節(jié)炎影響測量量表 2-短卷（AIMS2-SF）總分，外周血中性粒細胞與淋巴細胞比值（NLR）和血清甲殼質(zhì)酶蛋白40（YKL-40）、腫瘤壞死因子-α（TNF-α）、白細胞介素-18（IL-18）、前列腺素E₂（PGE₂）水平。結(jié)果 治療后，治療組總有效率為95.2%，較對照組的84.1%顯著升高（P＜0.05）。治療后，兩組關(guān)節(jié)疼痛VAS評分及關(guān)節(jié)腫脹、活動不利評分顯著降低（P＜0.05），且治療組的下降更顯著（P＜0.05）。治療后，兩組WOMAC總分均顯著低于治療前，而AIMS2-SF總分均顯著高于治療前（P＜0.05），且治療組的改善更顯著（P＜0.05）。治療后，兩組外周血NLR及血清YKL-40、TNF-α、IL-18、PGE₂水平均顯著下降（P＜0.05），且治療組較對照組降低更顯著（P＜0.05）。結(jié)論 復(fù)方玄駒膠囊聯(lián)合洛索洛芬鈉治療骨關(guān)節(jié)炎的療效確切，可安全有效地減輕患者關(guān)節(jié)疼痛、腫脹癥狀，改善關(guān)節(jié)功能，提高生活質(zhì)量。

Objective To investigate the clinical effect of Compound Xuanju Capsules combined with loxoprofen sodium in treatment of osteoarthritis.Methods Patients (126 cases) with osteoarthritis in First Teaching Hospital of Tianjin University of TCM from December 2019 to November 2020 were randomly divided into control (63 cases) and treatment (63 cases) group. Patients in the control group were po administered with Loxoprofen Sodium Tablets, 60 mg/time, three times daily. Patients in the treatment group were po administered with Compound Xuanju Capsules on the basis of the control group, 1.26 g/time, three times daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical evaluation was evaluated, the improvement of typical clinical manifestations, the total scores of WOMAC and AIMS2-SF, the levels of NLR of peripheral blood, and serum YKL-40, TNF-α, IL-18 and PGE₂ in two groups before and after treatment were compared.Results After treatment, the total effective rate of the treatment group was 95.2%, which was significantly higher than that of the control group (84.1%) (P < 0.05). After treatment, the VAS scores of joint pain and the scores of joint swelling and adverse activity in two groups were significantly decreased, especially in the treatment group (P < 0.05). After treatment, the total score of WOMAC in two groups was significantly lower than that before treatment, while the total score of AIMS2-SF in two groups was significantly higher than that before treatment (P < 0.05), and the improvement in the treatment group was more significant (P < 0.05). After treatment, the levels of peripheral blood NLR and serum YKL-40, TNF-α, IL-18 and PGE₂ in two groups were significantly decreased, and the levels in the treatment group were significantly lower than those in the control group (P < 0.05).Conclusion The curative effect of Compound Xuanju Capsules combined with loxoprofen sodium in treatment of osteoarthritis is definite, can safely and effectively relieve the symptoms of joint pain and swelling, improve joint function and improve the quality of life.
