目的 探討黃葵膠囊聯(lián)合硫唑嘌呤治療狼瘡性腎炎的臨床療效。方法 選取2019年1月—2021年12月百色市人民醫(yī)院腎內(nèi)科治療的72例狼瘡性腎炎患者，按照隨機(jī)數(shù)字表法將所有患者分為對(duì)照組和治療組，每組各36例。對(duì)照組患者口服硫唑嘌呤片，100 mg/次，1次/d。治療組在對(duì)照組的治療基礎(chǔ)上口服黃葵膠囊，5粒/次，3次/d。兩組均連續(xù)治療8周。觀察兩組的臨床療效和癥狀好轉(zhuǎn)時(shí)間。比較兩組尿蛋白、腎功能指標(biāo)、血清炎癥因子水平。結(jié)果 治療后，治療組總有效率是97.22%，顯著高于對(duì)照組的75.00%（P＜0.05）。經(jīng)治療，治療組出現(xiàn)發(fā)燒、關(guān)節(jié)痛、血尿、肢體浮腫等癥狀好轉(zhuǎn)時(shí)間均顯著短于對(duì)照組（P＜0.05）。治療后，兩組24 h尿蛋白定量（24 h-Upro）、尿微量白蛋白（mALB）、血肌酐（Scr）、血尿素氮（BUN）水平均較治療前顯著降低（P＜0.05）；治療后，治療組尿蛋白、腎功能指標(biāo)改善優(yōu)于對(duì)照組（P＜0.05）。治療后，兩組患者白細(xì)胞介素-6（IL-6）、白細(xì)胞介素-18（IL-18）、腫瘤壞死因子α（TNF-α）、C反應(yīng)蛋白（CRP）水平均較治療前顯著降低（P＜0.05）；治療后，治療組血清炎癥因子低于對(duì)照組（P＜0.05）。結(jié)論 黃葵膠囊聯(lián)合硫唑嘌呤治療狼瘡性腎病具有較好的臨床療效，可明顯減輕患者臨床癥狀，降低腎功能指標(biāo)和尿蛋白水平，降低機(jī)體炎癥反應(yīng)，值得臨床借鑒。

Objective To investigate the clinical effect of Huangkui Capsules combined with azathioprine on lupus nephritis.Methods A total of 72 patients with lupus nephritis treated in the Department of Nephrology, People’s Hospital of Baise from January 2019 to December 2021 were selected, and all patients were divided into control group and treatment group according to random number table method, with 36 cases in each group. Patients in the control group were po administered with Azathioprine Tablets, 100 mg/time, once daily. Patients in the treatment group were po administered with Huangkui Capsules on the basis of the control group, 5 grains/time, three times daily. Both groups were treated for 8 weeks. The clinical efficacy and symptom improvement time of the two groups were observed. The levels of urinary protein and serum factor of renal function were compared between the two groups.Results After treatment, the total effective rate of treatment group was 97.22%, significantly higher than 75.00% of control group (P < 0.05). After treatment, the improvement time of fever, arthralgia, hematuria, limb edema and other symptoms in the treatment group was significantly shorter than that in the control group (P < 0.05). After treatment, the levels of 24 h urinary protein quantification (24 h-Upro), urinary microalbumin (mALB), blood creatinine (Scr) and blood urea nitrogen (BUN) in two groups were significantly decreased compared with before treatment (P < 0.05). After treatment, the improvement of urinary protein and renal function in the treatment group was better than that in the control group (P < 0.05). After treatment, the levels of interleukin-6 (IL-6), interleukin-18 (IL-18), tumor necrosis factor α (TNF-α) and C-reactive protein (CRP) in two groups were significantly decreased compared with before treatment (P < 0.05). After treatment, serological indexes in the treatment group were lower than those in the control group (P < 0.05).Conclusion Huangkui Capsules combined with azathioprine has good clinical effect in treatment of lupus nephritis, and can significantly reduce the clinical symptoms of patients, reduce renal function indicators and urinary protein levels, and reduce the body’s inflammatory response, which is worthy of clinical reference.
