[摘要]
目的 探討重組牛堿性成纖維細胞生長因子凝膠聯合苯扎溴銨溶液治療小面積燒傷創(chuàng)面的臨床療效。方法 選取2019年4月—2021年4月在聊城市第二人民醫(yī)院治療的86例小面積燒傷患者,隨機分為對照組和治療組,每組各43例。對照組給予苯扎溴銨溶液,以濃度為0.01%的溶液清潔創(chuàng)面,2次/d;在此基礎上,治療組給予重組牛堿性成纖維細胞生長因子凝膠,每次300 IU/cm2涂于清創(chuàng)后的患處,2次/d。兩組均連續(xù)治療3周。觀察兩組患者臨床療效,比較治療前后兩組患者視覺模擬評分(VAS)評分、ADS-R評分和BSHS-B評分,臨床癥狀改善時間,創(chuàng)面肉芽組織中血管內皮生長因子(VEGF)、堿性成纖維細胞生長因子2(bFGF2)、缺氧誘導因子-1α(HIF-1α)和轉化生長因子-β(TGF-β)水平,創(chuàng)面愈合時間及換藥次數。結果 經治療,對照組臨床有效率為81.40%,顯著低于治療組(97.67%,P<0.05)。經治療,兩組VAS評分明顯下降,而傷殘接受度量表(ADS-R)評分和精簡燒傷健康量表(BSHS-B)評分明顯升高(P<0.05),且治療組患者評分明顯好于對照組(P<0.05)。經治療,治療組癥狀消失時間均早于對照組(P<0.05)。經治療,兩組創(chuàng)面肉芽組織中VEGF、bFGF2、HIF-1α、TGF-β陽性表達例數均明顯增多(P<0.05),且治療組明顯多于對照組(P<0.05)。經治療,治療組創(chuàng)面愈合時間明顯早于對照組,換藥次數明顯少于對照組(P<0.05)。結論 重組牛堿性成纖維細胞生長因子凝膠聯合苯扎溴銨溶液治療小面積燒傷創(chuàng)面效果良好,可促進創(chuàng)面肉芽組織生長,改善臨床癥狀,減輕疼痛及炎癥反應,促進創(chuàng)面愈合。

[Abstract]
Objective To investigate the clinical efficacy of Recombinant Bovine Basic Fibroblast Growth Factor Gel combined with Benzalkonium Bromide Solution in treatment of small area burn wounds. Methods Patients (86 cases) with small area burn wounds in the Second People's Hospital of Liaocheng from April 2019 to April 2021 were randomly divided into control and treatment group, and each group had 43 cases. Patients in the control group were administered with Benzalkonium Bromide Solution, the wound was cleaned with 0.01% solution, twice daily. Patients in the treatment group were administered with Recombinant Bovine Basic Fibroblast Growth Factor Gel on the basis of the control group, applied 300 IU/cm2 to the cleaned wound each time, twice daily. Patients in two groups were treated for 3 weeks. After treatment, the clinical evaluation was evaluated, the scores of VAS, ADS-R and BSHS-B, the disappearance time of clinical symptoms, the levels of VEGF, bFGF2, HIF-1α and TGF-βin granulation tissue, the wound healing time and dressing change times in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the control group was 81.40%, significantly lower than that of the treatment group (97.67%, P < 0.05). After treatment, the VAS score of the two groups were significantly decreased, while the ADS-R scores and BSHS-B scores were significantly increased (P < 0.05), and the scores of the treatment group were significantly better than that of the control group (P < 0.05). After treatment, the disappearance time of symptoms in the treatment group was earlier than that in the control group (P < 0.05). After treatment, the number of positive expression cases of VEGF, bFGF2, HIF-1α and TGF-β in wound granulation tissue of the two groups were significantly increased (P < 0.05), and which in the treatment group were significantly more than the control group (P < 0.05). After treatment, the wound healing time of the treatment group was significantly earlier than that of the control group, and the number of dressing changes was significantly less than that of the control group (P < 0.05). Conclusion Recombinant Bovine Basic Fibroblast Growth Factor Gel combined with Benzalkonium Bromide Solution is effective in treatment of small area burn wounds, which can promote the growth of wound granulation tissue, improve clinical symptoms, reduce pain and inflammatory reaction, and promote wound healing.
