目的 探討五味甘露藥浴顆粒聯(lián)合塞來昔布膠囊治療類風(fēng)濕關(guān)節(jié)炎的臨床療效。方法 選取2020年8月-2022年5月在焦作煤業(yè)集團(tuán)有限責(zé)任公司中央醫(yī)院就診的200例類風(fēng)濕關(guān)節(jié)炎患者,根據(jù)隨機(jī)數(shù)字表法將所有患者分為對照組和治療組,每組100例患者。對照組患者口服塞來昔布膠囊,1粒/次,2次/d。治療組在對照組治療的基礎(chǔ)上給予五味甘露藥浴顆粒治療,將1袋藥品倒入浴盆,水溫控制在40℃,將患病部位浸泡于浴盆內(nèi),1袋/次,1次/d,每次浸泡20 min。兩組患者連續(xù)治療3周。觀察兩組的臨床療效,比較兩組類風(fēng)濕關(guān)節(jié)炎病情評價量表(DAS28)評分、視覺模擬評分法(VAS)評分和血清類風(fēng)濕因子(RF)、C反應(yīng)蛋白(CRP)、抗環(huán)瓜氨酸肽(CCP)抗體水平。結(jié)果 治療后,治療組患者的總有效率為94.00%,對照組的總有效率為87.00%,組間比較有明顯差異(P<0.05)。治療后,兩組的DAS28評分、VAS評分低于治療前(P<0.05),且治療組DAS28評分、VAS評分明顯低于對照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組的血清RF、CRP、抗CCP抗體水平均顯著降低(P<0.05),治療組的血清RF、CRP、抗CCP抗體水平低于對照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 五味甘露藥浴顆粒聯(lián)合塞來昔布膠囊治療類風(fēng)濕關(guān)節(jié)炎的療效確切,能控制病情和減輕疼痛程度,降低RF、CRP、抗CCP抗體水平,并且安全性良好。

Objective To investigate the clinical effect of Wuwei Ganlu Medicated Bath Granules combined with Celecoxib Capsules in treatment of rheumatoid arthritis. Methods Patients (200 cases) with rheumatoid arthritis in Central Hospital of Jiaozuo Coal Industry Group Co., Ltd. from August 2020 to May 2022 were divided into control and treatment groups according to random number table method, and each group had 100 cases. Patients in the control group were po administered with Celecoxib Capsules, 1 grain/time, twice daily. Patients in the treatment group were given Wuwei Ganlu Medicated Bath Granules on the basis of the control group, poured a bag of medicine into the bathtub, control the water temperature at 40 ℃, and soaked the diseased part in the bathtub, 1 bag/time, once daily, soaked for 20 min each time. Patients in two groups were treated for 3 weeks. After treatment, the clinical efficacies were evaluated, and DAS28 scores, VAS scores, the levels of RF, CRP, and anti-CCP antibodies in two groups were compared. Results After treatment, the total effective rate of the patients in the treatment group was 94.00%, and the total effective rate of the control group was 87.00%, and there was a significant difference between two groups (P＜0.05). After treatment, the DAS28 score and the VAS score of two groups was lower than that before treatment (P＜0.05), and the DAS28 score and the VAS score of the treatment group was significantly lower than that of the control group, with a statistically significant difference (P＜0.05). After treatment, the serum levels of RF, CRP, and anti CCP antibodies in two groups were significantly decreased (P＜0.05). The serum levels of RF, CRP, and anti CCP antibodies in the treatment group were lower than those in the control group, with a statistically significant difference (P＜0.05). Conclusion Wuwei Ganlu Medicated Bath Granules combined with Celecoxib Capsules has definite therapeutic effect in treatment of rheumatoid arthritis, which can control the disease, reduce the degree of pain, and reduce the levels of RF, CRP, and anti-CCP antibody, with good safety.
