目的 探討益諾膠囊聯(lián)合卡前列素治療妊娠產(chǎn)后子宮出血的臨床療效。方法 選擇2019年1月-2022年1月在漯河市中心醫(yī)院診治的82例產(chǎn)后出血患者,隨機(jī)分對(duì)照組和治療組,每組各41例。對(duì)照組患者肌肉注射卡前列素氨丁三醇注射液,0.25 mg/次,1次/d。在對(duì)照組的基礎(chǔ)上,治療組口服益諾膠囊,2粒/次,2次/d。兩組治療5 d。觀察兩組患者臨床療效,比較治療前后兩組患者癥狀緩解時(shí)間,凝血功能指標(biāo)凝血酶原時(shí)間(PT)、凝血活酶時(shí)間(APTT)、纖維蛋白原(FIB)和D-二聚體(D-D),氧化應(yīng)激指標(biāo)丙二醛(MDA)、過(guò)氧化氫酶(CAT)和超氧化物歧化酶(SOD),及不良反應(yīng)。結(jié)果 治療后,治療組總有效率為97.56%,明顯高于對(duì)照組的78.05%(P<0.05)。治療后,治療組癥狀改善時(shí)間均明顯短于對(duì)照組(P<0.05)。治療后,兩組患者FIB、CAT、SOD指標(biāo)水平明顯升高,而D-D、PT、APTT和MDA指標(biāo)低于治療前(P<0.05),且治療后,治療組的凝血功能和氧化應(yīng)激指標(biāo)均明顯好于治療組(P<0.05)。治療組不良反應(yīng)發(fā)生率為7.32%,明顯低于對(duì)照組(17.07%,P<0.05)。結(jié)論 益諾膠囊聯(lián)合卡前列素氨丁三醇治療妊娠產(chǎn)后子宮出血療效較好,能有效止血,改善癥狀,緩解機(jī)體氧化應(yīng)激的能力。

Objective To investigate the clinical effect of Yinuo Capsules combined with carboprost in treatment of postpartum uterine bleeding during pregnancy. Methods Patients (82 cases) with postpartum hemorrhage in Luohe Central Hospital from January 2019 to January 2022 were randomly divided into control and treatment group, and each had 41 cases. Patients in the control group were intramuscular injection administered with Carboprost Tromethamine Injection, 0.25 mg/time, once daily. Patients in the treatment group po administered with Yinuo Capsules on the basis of the control group, 2 grains/time, twice daily. Patients in two groups were treated for 5 d. After treatment, the clinical evaluation was evaluated, the relief time of symptom, the levels of coagulation index FIB, D-D, PT and APTT, and oxidative stress response index MDA, CAT and SOD, and adverse reactions in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the treatment group was 97.56%, which was significantly higher than that of the control group (78.05%) (P＜0.05). After treatment, the time of symptom improvement in the treatment group was significantly shorter than that in the control group (P＜0.05). After treatment, the levels of FIB, CAT and SOD in two groups were significantly increased, while the indexes of D-D, PT, APTT and MDA were significantly lower than those before treatment (P＜0.05), after treatment, the indexes of blood coagulation and oxidative stress in the treatment group were significantly better than those in the treatment group (P＜0.05). The incidence of adverse reactions in the treatment group was 7.32%, which was significantly lower than that in the control group (17.07%, P＜0.05). Conclusion The combination of carboprost tromethamine and Yinuo Capsule is effective in stopping bleeding, improving symptoms and relieving oxidative stress.
